Head Injury Association is seeking full-time Day Habilitation Specialist to provide direct care to individuals supported in our day program located in Hauppauge. This position helps those we support in completing a variety of activities including cooking, computer lab, horticulture, photography and more. Essential Job Responsibilities

• Obtain and maintain certifications needed to fulfill job (AMAP, Diabetes, Gastrostomy Care, etc.)
• Perform AMAP responsibilities, adhering to AMAP policies distributing medications as indicated on MAR (physician’s orders).
• Receive, review for accuracy, and collate Medication Administration Records and orders from residential staff and/or Nursing Department.
• Maintain inventory of medication; request medication when two-week supply is being depleted and follow-up to ensure request is made to individual’s residence to ensure receipt.
• Prepares individual’s medication for off-site trips as needed, ensure rescue medications are signed out and travel with individual. Receive and sign in medications upon trip return.
• Ability to administer basic First Aid and emergency care as needed.
• Ability to complete vital signs, weight check or individualized flow sheet documentation as per physician’s orders.
• Report changes in medical status or acute concerns to Registered Nurse on shift as per protocol.
• Ability to participate in team meetings as requested by Nursing staff.

• Monday - Friday: 10:00am - 2:00pm

Education and Experience:

• High School Diploma or Equivalent
• AMAP certification, required
• 1 year of related experience
• Must be able to follow written and verbal instructions
