Hayward Convalescent Hospital is a 99 bed skilled nursing facility . We are looking for a motivated, detail oriented individual who is qualified to work as the Medical Records Clerk. The primary purpose of the job position is to assure that medical records are maintained in accordance with federal and state guidelines. The Medical Records department plays a major role in helping us to consistently deliver 5-star service to every resident that has the opportunity to come to our facility.

Administrative Functions:

• Assist in planning, developing, organizing, implementing, evaluating, and directing the medical records section in accordance with established policies and procedures.
• Assist in establishing procedures to be followed in the collection, coding and indexing, and the filing/retrieving of medical records. Review discharge and death records, as well as records on the nursing units.
• Collect, assemble, check, and file resident charts and personnel records as may be necessary.
• Ensure that incomplete records/charts are returned to nursing service for correction.
• Daily and consistent follow ups with Nursing staff on clinical documentation.

Job Qualifications:

• Must possess the ability to make independent decisions when circumstances warrant such action.
• Must possess the ability to deal tactfully with personnel, residents, visitors and the general public.
• Must be knowledgeable of medical terminology, and regulations as they pertain to long-term care.
• Be knowledgeable in micro-computers, data input and output.
• Knowledgeable with HIPPAA Guidelines

Job Type: Full-time

Pay: $17.00 - $20.00 per hour

Expected hours: 40 per week
