DUTIES AND RESPONSIBILITIES:
1. Organize and direct the Haven Home Health’s on-going functions
2. Ensure compliance with all local, state and federal laws and regulations
3. Maintain on-going liaison with the governing body/owners, staff members and the community
4. Maintain a current organizational chart
5. Assure hiring of qualified personnel and ensures adequate staffing, on-going education, training and evaluations for the staff
6. Maintain the policy and procedure manual; review/update on at least on an annual basis
7. Ensure accuracy of Haven Home Health’s public informational materials and activities
8. Demonstrate revenue cycle efficacy towards Haven Home Health and organizational metric goals
9. Complete, maintain and submit all reports required by federal, state and local regulations
10. Designate, in writing, a qualified staff member to act in the Administrator/DON’s absence
11. Provide a system of staff communications that ensures coordinated implementation of plan of cares and the utilization of necessary services for the comprehensive care of patients
12. Comply with HIPAA privacy rules, policies and procedures and may serve as the privacy officer
13. Maintain and conduct self in a professional manner at all times and situations
14. Maintain strict confidentiality of all patient, employee and Haven Home Health information
15. Report and document any suspected violations of the Privacy Practice to the privacy official as soon as breach/violation is identified
16. Responsible for all day-to-day operations of the HHA
17. Ensure that a clinical manager is available during all operating hours
18. When the administrator is not available, a qualified, predesignated person who is authorized in writing by the administrator and the Governing Body assumes the responsibilities and obligation as the administrator. The predesignated person may be the clinical manager (RN)
19. The administrator or predetermined person is available during all operating hours
20. Identify and evaluate the potential resource needs in relation to community health and the Haven Home Health’s policies and procedures
21. Assist in the development of the policies, programs and functions for the Haven Home Health and review/update at least annually
22. Participate in the review, analysis and appraised effectiveness of the total Haven Home Health program
23. Responsible to ensure orientation of all new staff is carried out prior to the first case assignment
24. Define, allocates and supervise responsibilities within the province of nursing
25. Define and prepare job classifications and job descriptions of nursing service personnel and qualifications within the nursing program
26. Recruit and maintain RN’s. LPN’s, HHA’s, FHCA’s and other clinical staff as needed, as additional positions arise or as vacancies occur in order to maintain sufficient qualified staff to safely and efficiently perform clinical functions
27. Oversee all PI/OBQI/OBQM activities and participates in all activities relevant to the professional services furnished
28. Provides for continuing evaluation of the nursing staff by:
a. Evaluating own job performance and utilizing available help in planning for professional growth
b. Evaluating the performance of other individuals in the division in relation to established standards and the individual’s professional development
c. Evaluating total nursing services in relation to Haven Home Health goals
29. Participates in quarterly meetings with the National/Regional Director of Clinical Operations to ensure optimum communication interdepartmentally and to discuss any mutual or Haven Home Health problems.
30. Direct and coordinate services provided by the contracted personnel
31. Review all logs, including but not limited to, infection control, patient complaints, and incident reports on at least a quarterly basis, investigate problems/potential problems noted and offer recommendations for resolution/change
32. Available at all times except with approved time off in the absence of the Administrator/DON
33. Responsible to compile a daily report of health services provided by a direct employee or contracted staff member
34. Provide initial and on-going assessment of client needs using the OASIS data elements incorporated into the comprehensive assessment
35. Formulate and implement plan of care with input from patient/caregiver and in conjunction with the physician
36. Evaluate effectiveness of care plan and make necessary adjustments
37. Provide for the emotional and physical comfort and safety of client taking into consideration their rights and cultural background
38. Receive and transcribe physician orders
39. Notify physician of unusual reactions and/or changes in client’s condition
40. Document all appropriate observations and treatments in keeping with Haven Home Health’s policies and procedures
41. Schedule and attend case conferences, team meetings, staff meetings and performance improvement activities
42. Provide any skilled nursing service for which appropriately trained which is prescribed under the plan of care. Prepare clinical progress notes as per Haven Home Health policies and procedures
43. Ensure all staff are up to date on RELIAS (continuing education) and all HR related items
44. Other duties assigned by supervisor by which are qualified
Salary Range: $90,000 to $98,000 per year depending on experience plus $50 per month cell phone reimbursement, hourly rate for bedside hours worked and mileage reimbursement for company travel
