Responsibilities

The Palliative Care Medical Social Worker (PCMSW)is a qualified professional who, in accordance with the plan of treatment, assists the Palliative Care Nurse Practitioner and other team members in understanding the significant social and emotional factors related to the patient health concerns. The PCMSW works collaboratively with the Care Team and the patient/caregiver to provide care management services that address an individual’s ability to maintain independence in the home, chronic care management, end of life decisions, cognitive/functional status, frequency of acute care utilization, quality of life and a patient/caregiver support system.

Essential Duties and Responsibilities

  • Provide social casework as an entity, where appropriate, to individuals and families who have been referred by the Palliative Care Nurse Practitioner or Care Team
  • Conduct face to face and telephonic assessments with the patient and/or the caregiver
  • Prepare social histories according to specific guidelines to augment existing service or as a guide in determining or changing level of service taking into consideration:
    • Family relationships
    • The meaning of illness to the patient and his family
    • The cultural attitudes in relationship to illness
    • The meaning of home to the patient
    • The ability of the patient and his family to adjust to the patient at home
    • The financial impact of illness upon the family
  • Completes clinical and progress notes
  • Investigate social determinant barriers and design appropriate interventions
  • Coordinate services to ensure the patient/family understands the treatment plan
  • Provide outreach to community based services to support the patient’s ability to age at home
  • Develop and implement shared goals of care with the patient and family
  • Participate in Interdisciplinary Group team Meetings
  • Maintain absolute confidentiality of patient records
  • Participate in plan of care and discharge planning
  • Assess, when appropriate, a family's financial situation taking into consideration the patient's prognosis and medical needs, and referring to an Agency for financial assistance if indicated, interpreting the medical situation to the referring Agency and generally facilitating the process of referral
  • Observe, record, and report information on the patient's condition to the Palliative Care Nurse Practitioner and Care Team
  • Participate in in-service education programs
  • Will understand and apply basic infection control principles and procedures
  • Performs additional duties as assigned

Qualifications

Required Knowledge, Skills and Experience

  • Master's degree (MSW) from a school of Social Work accredited by the Council on Social Work Education
  • Has a current Certified Master Social Worker (CMSW) License in the state of practice in accordance with state licensure requirements
  • Must maintain a valid driver’s license and good driving record
  • Proficiency in clinical skills
  • The ability to make sound professional clinical judgment
  • The ability to assess and document patient needs and formulate individualized patient care plans to meet those needs
  • Excellent verbal and written communication skills
  • Proficient in personal computer use including email, clinical, word processing, and spreadsheet software

Additional Florida Requirements

  • Active CPR Certification

Preferred Knowledge, Skills and Experience

  • Advanced Palliative Hospice Social Worker Certification-APHSW-C, through Social Work and Palliative Care Network
  • Two years’ experience within Palliative Care
