Hampton Roads Regional Jail Authority is Hiring a Serious Mental Illness Monitor & Advocate (FT) Near Portsmouth, VA

Hampton Roads Regional Jail has implemented a team to educate and meet the needs of our SMI population while maintaining public safety. The role of the “SMI Monitor & Advocate” is to provide strong provider-patient communication with individuals who have one or more mental, behavioral, or emotional disorders resulting in serious functional impairment, which substantially interferes with or limits one or more major life activities.
The SMI-M&A seeks to understand, develop rapport, and engage SMI’s as outlined to attain medication compliance which can be an antecedent of patients’ health outcomes. This role will participate with jail safety, security medical and mental health staff regarding nonadherence to medication which could result in poor health outcomes, including worsening medical conditions, an increase in comorbidities, and death.
The overall goal of Hampton Roads Regional Jail SMI Monitor & Advocate is to assist those inmates with SMI by daily observation and communication with them, providing necessary attention they require as it pertains to, addressing their needs and advocating for them. The SMI-M&A will offer solutions based on their understanding of the influencing factors pursuant to providers, patients, and medications in the HRRJ jail environment.
THIS ROLE WILL INCLUDE THE FOLLOWING:
  • Serves as an SMI advocate consultant to Administration, Security, Treatment and Community Corrections staff.
  • Provide monitoring and consultative services to staff and management to ensure medication compliance is adhered to.
  • Responsible for monitoring; this entails a daily presence throughout the facility with noted concerns being promptly addressed or communicated to facility management.
  • Educate patients/consumers on caseload through empowering them to self-advocate as they are able.
  • Attends daily whiteboard meetings to gather the prevalent needs of the SMI patient.
  • Attends weekly ICC meetings to engage, monitor and advocate for those on the SMI list who have one or more mental, behavioral, or emotional disorders resulting in serious functional impairment, which substantially interferes with or limits one or more major life activities.
  • May be involved with special units or programs.

Reports directly to the Director to ensure data of medication compliance with SMI caseload is accurate in addition to reporting the below:
  • Documents clients contact through weekly log of updates and shares with Director on a weekly basis
  • Responds efficiently to items and data requested by Director for the Director to report out to HRRJ administration and the Jail Board Authority
  • Conducts accurate counts of caseload and reports any emergency situations to Jail Administration to include Chief of Security
  • Keeps an accurate update and knowledge of logged medications of SMI caseload
  • Assist the Director in communication with local stakeholders to include public health education
  • Assists the Director with tracking outcomes that contribute to the annual end of the year reporting and outcomes.
  • Assist the Director with implementing administrative goals/standards through reviewing, evaluating, identifying, and resolving system problems related to medication management occurrences with SMI population and CMS guidelines.
  • Assist the Director with In-Service Trainings and education to facility staff
  • Assist the director in the development, implementation and monitoring of mental health services, policies, and programs at the facility.
  • All trainings of interest must be approved and signed off by direct supervisor on 225 updated forms
  • Reports critical mental health needs immediately to Director and Chief of Security as soon as possible.
  • Corresponds with the Medical Contract Staff Wellpath to include receiving SMI patient diagnosis and medication list to ensure follow-up and compliance.
  • Supervises face to face interaction with SMI population that have an urgent need to maintain their therapy regimen even if they have a change in medication.
  • Work includes collaboration with the Chief of Security and Captains when there is alleged or suspected neglect, abuse, exploitation, disagreement of services provided violations of patients/residents/consumers.
  • Responds to Kiosk request of those assigned to SMI caseload and those assigned by Jail Leadership staff.
  • Maintains constant and positive interactions with the population of heightened emotional disturbances and reports the same up the chain of command.
  • Rapid turn-around and follow up with inmate upon return from Eastern State Hospital which includes collaboration with Integrated Care Management team at Eastern State and any Psychiatric or hospital facilities where the inmate is housed while in custody of HRRJ.
  • Collaboration and rapport with Psychiatry, DON -Director of Nursing, Mental Health staff not to exclude LPN, RN, and Med Tech staff to ensure follow-up of medication management of the caseload provided.
  • As a QMHP-A understands the obstacles patients face while obtaining and taking medication is essential. This individual must be able to provide ongoing education to caseload during one-to-one follow-ups and participate in therapeutic groups facilitated by Program Staff and Jail Mental Health staff (Wellpath) in the facility- as needed.
  • Ability to navigate through patient/caseload medical records and jail technology systems (JMS and Electronic Health Medical Records) and maintain PHI and HIPPA privacy.
  • Participate in a collaborative approach to medicine adherence which includes outstanding communication skills and delivery of non-conflicting information.
  • Address barriers between patients and providers pursuant to medications and advocate for the patient’s ability to adhere to medication. These barriers may include but are not limited to during incarceration: violence, diet of food, participation in physical activity, literacy, and language.
  • Access side effects related to treatment related barriers such as side effects, time of medication, complexity and obstacles and report that to medical staff.
  • Streamline with Jail leadership and medical staff regarding reduced treatment efficacy, hospital visits and adverse events of the SMI patients involved on the caseload.
  • Tracks and monitors offenders with mental health services needs in intake, general population, and special housing.
THIS ROLE DOES NOT:
  • This role does not provide patient care such as administering medication and treatments.
  • This role does not maintain patient medical records.
  • This role does not provide legal advice or court advocacy.
  • This role does not conduct discharge planning but will correspond with SMI population that is about to be released that will need coordination of care regarding medication compliance follow-up. This includes following up with the patient up to 30 days post-release and 30 days prior to release to ensure they have received their medication through Wellpath, local CSB or through their pharmacy of choice.
  • This role does not conduct TDOS or Certified Pre-Screenings
This role promotes health and wellness education and communicating for continuity of care.
This is a full-time position and eligible for the full State benefits package, including retirement and health insurance. The normal work hours are Monday - Friday, 8:00am - 4:30pm; however, flexibility is available of a mid-day shift to include 11am-7pm.
REQUIRED QUALIFICATIONS:
  • Graduation with a bachelors from an accredited college or university in Criminal Justice, human services field (including, but not limited to sociology, behavioral health / analysis, special education, rehabilitation counseling, and psychology) and/or equivalent combination of education and experience preferred.
  • Candidate is required to have a Qualified Mental Health Certification in the Commonwealth of Virginia
  • Current CPR and First Aid certification.
  • Skill in the use approved Department of Behavioral Health and Developmental Services intervention techniques, restraints, and electronic controls and locking systems is required
  • Experience working directly with individuals with challenging mental health and behavior issues in a public, residential, or psychiatric hospital preferred.
  • Experience in a Law Enforcement, Security, Corrections, or military background is required
  • Demonstrated ability and skills in providing facility psycho-educational groups
  • Knowledge, skills, and ability to communicate effectively orally and in writing (basic word processing and computer skills)
  • Ability to use good judgement
  • Ability to interact with other professionals from various disciplines
PREFERRED QUALIFICATIONS:
  • Working knowledge of diagnosis and medication management.
  • Demonstrated ability to communicate effectively with patients, families, staff, and other professionals.
  • Specialized training in the management of aggressive mental health behavior preferred.
  • Ability to documents findings accurately and thoroughly, to recognize legal limits of practice, to obtain/interpret clinical data and draw satisfactory conclusions and to access emergency situations and take appropriate actions.
  • Prefer relevant professional experience and understanding of confidentiality of sensitive and protected offender information.
