Reinvestment Case Manager - State Hospital Discharge Planning Team

Salary: $47,250

Work Schedule: Monday - Friday 8:30 am - 5:00 pm

Are you passionate about supporting individuals in their journey towards recovery and reintegration into the community? Do you have experience in case management services and a desire to work with individuals facing mental health challenges? If so, we invite you to apply for the position of Reinvestment Case Manager with the H-NNCSB State Hospital Discharge Planning Team.

Our State Hospital Discharge Planning team is dedicated to providing exceptional case management and discharge planning services to individuals admitted to State psychiatric facilities. Comprised of compassionate professionals including Discharge Planners and Case Managers, we advocate for individual needs, monitor treatment progress, and coordinate seamless transitions to community-based care.

We serve a diverse population of adults aged 18 and older, including those with serious mental illnesses, intellectual or developmental disabilities, substance use concerns, and individuals with Not Guilty by Reason of Insanity (NGRI) status. With our expertise and commitment to collaboration with legal and mental health providers, we strive to develop tailored discharge plans that support individuals' recovery and successful reintegration into the community.

Since 1971, the Hampton-Newport News Community Services Board (H-NNCSB) has been at the forefront of delivering comprehensive, high-quality services to citizens affected by mental illness, developmental disabilities, and substance use disorders on the Peninsula.

As a Reinvestment Case Manager, you will play a vital role in providing short-term case management to individuals aged 18 and older who are admitted to local psychiatric hospitals. Your responsibilities will include discharge planning, linking individuals to CSB and community resources, and ensuring continuity of services to our client populations.

Key Responsibilities:

- Conduct outreach and assessments to identify individual needs and develop comprehensive treatment plans.
- Link individuals to appropriate community resources and services, ensuring continuity of care upon discharge.
- Provide ongoing monitoring, advocacy, counseling, and crisis intervention as needed.
- Support clients with daily living activities, medication supervision, and other essential needs.
- Collaborate closely with interdisciplinary teams, legal professionals, and mental health providers to develop and implement effective discharge plans.
- Maintain accurate documentation and records of client interactions and progress.

Requirements:

- Bachelor's degree in Human Services (Social Work, Psychology, Sociology, or Counseling) with at least one (1) year of experience in case management services.
- Valid Driver's License.
