Hampton-Newport News Community Services Board is Hiring a Case Manager - Adult Mental Health Near Hampton, VA

Schedule: Monday-Friday 8:30 am - 5:00 pm

The Hampton - Newport News Community Services Board is a dedicated provider of comprehensive mental health services with a mission to support individuals with Serious Mental Illness or Serious Mental Illness and Co-occurring Substance Use Disorders in achieving and maintaining optimal functioning within the least restrictive, most normative community setting. Our approach involves assessing individuals' needs and strengths, linking them with community services and supports, and closely monitoring their response to treatment and the quality of care provided. We place great emphasis on tailoring our services to individual needs, always mindful of cultural values and preferences. The individuals we serve often have complex behavioral health needs requiring multiple services and supports.

As an Adult Mental Health Case Manager, you will play a pivotal role in supporting individuals with Serious Mental Illness. Your primary responsibility will be to assess their unique needs, strengths, and goals, and work collaboratively to help them achieve optimal functioning within the community. You will be at the forefront of crafting individualized care plans and connecting individuals to a network of community services and supports.

Key Responsibilities:

  • Conduct comprehensive assessments of individuals' mental health needs and strengths.
  • Develop and implement individualized care plans in collaboration with clients.
  • Link individuals to appropriate community services and resources.
  • Monitor and evaluate the quality of care provided and the effectiveness of treatment.
  • Provide ongoing support, advocacy, and crisis intervention as needed.
  • Maintain accurate and up-to-date case records and documentation.
  • Collaborate with a multidisciplinary team of professionals to ensure holistic care.

Requirements: Bachelor's degree in human services, such as Social Work, Psychology, Sociology, or Counseling, with one (1) year of experience in case management services. Valid Driver's License.

The selected candidate must successfully pass a criminal history fingerprint background investigation, DMV record check, Child Registry search, drug screening test and employment reference checks.
