Case Manager- Hampton Healthy Families

Annual Salary: $47,250

Schedule: Monday-Friday 8:30 am - 5:00 pm

The Hampton - Newport News Community Services Board and the Hampton Healthy Families program offers Healthy Families America Home Visiting Services in combination with Mental Health Case Management services, to some of the most vulnerable young children and families in our community. The Healthy Families America evidence-based curriculum complements the linkage, coordination, and advocacy available as part of Case Management services. These services focus on strengthening nurturing parent-child relationships and promoting healthy childhood growth and development.

The Hampton Healthy Families Case Managers assist clients with their basic physical, financial, social and personal needs. Healthy Families case management services for the client include outreach, psychological and need assessments, treatment planning and service provider linkage, monitoring, advocacy, counseling and crisis intervention, daily living support, medication supervision, and similar functions.

• Clinical Assessment and Referrals
• Linkage and Coordination of Services
• Implementation of the Healthy Families of America Curriculum to promote healthy childhood and development

Case Manager Requirements: Bachelor's degree in human services (such as: Social Work, Psychology, Sociology, or Counseling) with one (1) year experience in case management services for children and adolescents. Valid Driver's License

The selected candidate must successfully pass a criminal history fingerprint background investigation, DMV record check, Child Registry search, drug screening test and employment reference checks.
