As we continue to grow, we are seeking a dedicated Patient Care Coordinator to join our team and provide essential administrative support to our interdisciplinary care team.

Job Summary:

As a Patient Care Secretary at Halo Hospice, you will serve as a vital point of contact for patients, families, and healthcare professionals, ensuring smooth coordination of care and administrative operations. You will play a key role in facilitating communication, managing records, and providing exceptional customer service to all stakeholders. We are looking for a detail-oriented individual with excellent organizational skills and a compassionate demeanor to join our team and contribute to our mission of providing dignified and personalized care to those we serve.

Responsibilities:

* Greet visitors, answer phone calls, and respond to inquiries from patients, families, and healthcare providers in a professional and courteous manner.
* Coordinate patient admissions, discharges, and transfers, ensuring accurate and timely documentation of all related activities.
* Schedule appointments, arrange transportation, and assist with coordinating services such as durable medical equipment and medication delivery.
* Maintain electronic medical records (EMR) and ensure accuracy, completeness, and confidentiality of patient information in compliance with HIPAA regulations.
* Assist with billing and insurance-related tasks, including verifying insurance coverage, obtaining authorizations, and processing claims.
* Collaborate with the interdisciplinary care team to ensure seamless communication and coordination of care for patients and families.
* Prepare and distribute correspondence, reports, and other documents as needed, both internally and externally.
* Assist with organizing and conducting team meetings, in-service training sessions, and other events as directed.
* Provide administrative support to the Director of Patient Care Services and other members of the leadership team as needed.

* Assist with preparation of the IDT meeting.

Requirements:

* High school diploma or equivalent required; associate's or bachelor's degree preferred.
* Previous experience in a healthcare or hospice setting preferred, with knowledge of medical terminology and procedures.
* Proficiency in computer applications such as Microsoft Office Suite and electronic medical record (EMR) systems.
* Strong organizational skills and attention to detail, with the ability to prioritize tasks and manage multiple responsibilities effectively.
* Excellent interpersonal and communication skills, with a customer service-oriented approach.
* Ability to maintain confidentiality and exercise discretion in handling sensitive information.
* Flexibility to adapt to changing priorities and work independently as well as part of a team.
* Compassionate and empathetic demeanor, with a commitment to providing exceptional care and support to patients and families during difficult times.
