Job Summary:
The Center Manager is responsible for the center’s day-to-day operations and coordinating all care delivered to Participants. This includes the management of the Interdisciplinary Care Team as well as the oversight of the facility’s operations and systems.
Duties/Responsibilities:
  • Coordinate all Center activities, including care planning and implementation, preparation for the interdisciplinary conference and daily meetings, family conferences, and all related follow-ups.
  • Responsible for human resource management of all disciplines, including RNs, LPNs, therapists, clerical staff, NTs, MSWs, and transportation personnel.
  • Serve as liaison between individual program disciplines and community service providers.
  • P&L responsibilities.
  • Coordinates all activities relating to new enrollees.
  • Communicates Participant changes to team members and families.
  • Participates in the development of Care Plans.
  • Participates in the coordination of a 24-hour care delivery.
Education and Experience Requirements:
  • Bachelor’s degree in a related field with at least one year of experience working with the elderly.
  • Experience working in a nursing home, at-home care, or day service provider is required.
Required Skills/Abilities:
  • Must be able to deal tactfully and patiently with personnel, Participants, family members, visitors, government agencies/personnel, and the public.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Excellent written and verbal communication skills.
  • Must be able to complete a full background check.
