Gurwin Jewish Nursing & Rehabilitation Center is a renowned healthcare provider located on Long Island, NY which offers a full continuum of services. The Nursing & Rehabilitation Center is a 460 bed facility providing services for short-term rehabilitation and long-term skilled nursing care, home care programs, an adult day health program, ventilator dependent care, palliative and hospice care, a new Memory Care Unit and on-site dialysis. The Gurwin Family of Services also includes the 201-apartment Gurwin Jewish ~ Fay J. Lindner assisted living community, and the Fountaingate Gardens independent living community on the Commack campus.

We are currently seeking a part time or full-time Employee Health Nurse with days worked Monday through Friday. The appropriate candidate will embody the passion and commitment required to deliver the ultimate care for our residents.

EDUCATION REQUIREMENTS:

• Must be a Master’s Degree graduate of an accredited Nurse Practitioner program and currently be registered in the State of New York.
• Prior employee health experience preferred.

DUTIES AND RESPONSIBILITIES:

• Provides initial physical assessments of employees and contractors for all Gurwin entities. Conducts annual and pre-employment physicals.
• Oversees scheduling and administration of hepatitis, influenza and pneumococcal vaccines.
• Ensures proper consent is on file.
• Draws blood for measles, rubella and hepatitis titers.
• Administers PPD’s, tracks results. Completes PPD counseling on all employees hired with a history of positive PPD and on any employee who converts to positive while employed. Determines need for further testing and medical clearance.
• Informs employees of abnormal test results and follows through.
• Maintains employee health records for Gurwin entities detailing problems, findings, evaluations and dispositions in a confidential manner. May refer employees with medical issues to appropriate referral source.
• Reviews health files on all new employees and contractors.
• Participates, supports and implements continuous quality improvement and quality assurance which is both facility-wide and medical department based.
• Works cooperatively with Infection control RN regarding infection control issues.
• Counsels employees who report exposure to blood borne pathogens and completes appropriate paperwork as per policy
• Reviews medical clearances for Fountaingate prospects.
• Contacts employees on leave of absence to evaluate their status and ensure a timely clearance to return to work.
• Maintains confidentiality of employee medical records/information at all times.
• Demonstrates an attitude of cooperation and professionalism when dealing with peers and supervisors.
• Responsible for regulatory reporting such as COVID and CDC.
