The Nursing & Rehabilitation Center is a 460 bed facility providing services for short-term rehabilitation and long-term skilled nursing care, home care programs, an adult day health program, ventilator dependent care, palliative and hospice care, a Memory Care Unit and on-site dialysis. EDUCATION/EXPERIENCE REQUIREMENTS INCLUDE: High School Diploma, minimum 2 years SNF, DSS or related experience.
JOB DESCRIPTION:
Meet with families of prospective residents prior to admission. Collect and review all documentation required to file Medicaid application. Advise prospective resident family members regarding Medicare and Medicaid regulations, facility policy, and to answer all questions. Investigate and process Medicaid Conversions, Recertification’s, Applications, Provider Changes, and monthly Roster’s for all counties.
