Job Title: Case Manager-ACT/Criminal Justice
Location: Office in Texas City, TX. Serves Brazoria and Galveston Counties
FLSA Status: Non-Exempt
Working Status: Full Time with Benefits
Summary:
The ACT Criminal Justice Rehabilitative Case Manager (RCM) position will be responsible to work directly with the Galveston County jail in order to identify potentially ACT qualifying individuals in the county jail that are in need of engagement and specialized ACT intensity services to avoid jail recidivism.
The ACT Criminal Justice RCM will carry a caseload of 10 mentally ill high-utilizers of the Galveston County jail providing enhanced engagement and specialized ACT intensity case management and rehabilitative services to reduce their risk of jail recidivism. The target population will be ACT eligible individuals that have had two or more bookings to the Galveston County jail in the last year. This position will also provide specialized services to current ACT customers that become justice-involved in order to address their special needs and to help them avoid arrest.
ACT is an integrated service approach merging clinical and rehabilitation staff expertise within a mobile service delivery team that works in partnership with the person in recovery from his or her home. ACT teams are designed to provide comprehensive psychiatric treatment, rehabilitation and support to persons with serious and persistent mental illness to live independently in the community. The goal is to provide an intensive level of services that will reduce recidivism and increase one's tenure in the community. With introduction to mental health services on the intensive level, those served will receive frequent contact to facilitate lasting engagement to services, linkage to housing services, benefits using an integrated team approach to decrease the frequency of corrective system re-entry.
The ACT Criminal Justice RCM will work with jail referrals that are not ACT qualifying to refer them to other appropriate services.

The Case Manager-ACT/Criminal Justice will be responsible for the following essential Job Duties:
  • Identifies a current and discreet caseload as part of an interdisciplinary team.
  • Serve as a member of the treatment team coordinating and providing services as needed.
  • Actively participate in team meetings as evidenced by discussion that is not only specific to assigned caseload.
  • Display professionalism and team membership when representing program.
  • Personnel vacancy may result in temporary increase of caseload.
  • Maintain contact with designated staff on a daily basis regarding current work area and retrieval of messages.
  • Provides RCM self management strategies and clinical coordination to individuals, groups, and family members as defined by GCC using evidence based strategies within the (TRR) model of care.
  • Provides crisis services to consumers and walk-ins to the clinic as needed.
  • Provides crisis services for after hours response including nights, weekends, and holidays to ACT clients per rotation among all staff on the ACT team.
  • Provides monthly documented face to face hours of direct service per month to GCC consumers and/or family members according to the current Direct Service Agreement.
  • Monitors consumer' requirements within Disease Management Model to ensure required services are offered including 80% minimum service hours are met for identified caseload.
  • Assure treatment provided to consumer relates to the individualized goals of treatment plan.
  • Utilizes a variety of skills training in conjunction with the IMR (Illness Management and Recovery Model) and may utilize the following intervention strategies: instruction, positive feedback, repetition, role play, behavioral reversal, in-vivo practice and coaching.
  • Responsible to follow up on the potential ACT qualifying referrals within 2 business days to determine eligibility.
  • Responsible to increase enrollments of ACT eligible high-utilizers of the Galveston County jail. Will serve an average of 10 unduplicated and 70 duplicated individuals a month and 20 total unduplicated individuals for the fiscal year.
  • Expected outcome will be that ACT served individuals with 90 days or more in project services will have reduced risk for jail recidivism as measured by stable or improving scores on the "Criminal Behavior" and "Arrests" dimensions of their Adult Needs and Strengths Assessment (ANSA).
  • Enters into EHR, accurate and appropriate documentation within 2 business days of services per TAC and GCC documentation policy requirements
  • Maintains compliance with all documentation for consumers as assigned and as required by the GCC policies and procedures within caseload.
  • Conducts the Adult Needs and Strengths Assessment (ANSA) every 6 months or as clinically indicated for individuals as required and assigned. Responsible for ensuring the completion of the initial and annual ANSA recertification pursuant to the Praed Foundation and HHSC contract. Any lapse in certification will result in formal disciplinary action. Exception: If certification lapse/expiration is reported to supervisor within 24 hours of expiration date, employee will be granted opportunity to recertify by close of following business day. Any lapse/expiration greater than 24 hours will result in termination of employment Additionally, all certified practitioners are responsible for participating in annual quality assurance training activity performed by the identified ANSA super user.
  • Participate in Medicaid Administrative Claiming process.
  • Through a system of care, helps to establish baseline data & assists with tracking vital health indicators as a part of CCBHC & 1115 Waiver outcome measures
  • Will be responsible and accountable for the adherence to and promotion of the agency compliance policy and will require communication with various departments to foster, develop and continue our community partnerships as it relates to the position.
  • Treat all individuals with the utmost courtesy, decency and respect.
  • Provide responsive, and efficient services.
  • Interact with staff, consumers, local entities and advocacy groups to provide a service that meets local needs.
  • Provide highest level of Customer Service for both internal and external customers.
  • Required to report personnel leave; flex time and emergency leave to assigned supervisor.
  • Strict adherence to the professional, ethical, and legal requirements set forth in GCC's Business Code of Conduct as well as the Corporate Compliance Plan.
  • Consistent adherence to the Center's Dress Code.
  • Consistent adherence to Center safety guidelines and expectations.
  • May be required to travel within Brazoria and Galveston Counties.
  • Out of region travel may be required to attend trainings.
  • Must maintain a clear driving record and driver's license in accordance with our Driver Policy.
  • Requires current Texas Driver's License, reliable transportation and must maintain current auto liability insurance.
Supervision:
  • Receives direct supervision from Program Manager-ACT
Required Education:
  • Requires a bachelor's degree or equivalent in the behavioral sciences field of study from an accredited college or university and/or specialized QMHP certification.
Required Experience:
  • Minimum one year related work experience with behavioral health services
Preferred Experience:
  • Experience with the criminal justice system

Working Conditions:

  • Out of region travel may be required to attend trainings.
  • May require travel to various locations within Brazoria and Galveston Counties. Requires current Texas Driver's License, reliable transportation safe and current with all state requirements, and liability insurance.
  • Majority of work is in the office and/or community.
  • May periodically transport consumers in personal and/or Gulf Coast Center Vehicle.
  • May be asked to participate in after hours on-call rotation
Special Projects:
  • Assist supervisor when requested to complete special projects and complete all assignments within a time frame negotiated with the supervisor. Completion of other duties as assigned by supervisor as it relates to the program and position.
