Job Overview:
We are seeking a compassionate and skilled Hospice Director of Nursing to join our team. As a DON, you will provide comprehensive nursing care to patients in their homes or Nursing facilities. You will work closely with patients, their families, and a multidisciplinary team to ensure the highest quality of care and comfort during end-of-life stages.
Duties:
- Conduct initial assessments of patients' medical conditions and develop individualized care plans
- Administer medications and treatments as prescribed by physicians
- Monitor patients' vital signs and symptoms, and provide appropriate interventions
- Collaborate with physicians, social workers, and other healthcare professionals to coordinate patient care
- Educate patients and their families on disease management, symptom control, and end-of-life care options
- Provide emotional support to patients and their families throughout the hospice journey
- Document patient care activities, including assessments, interventions, and outcomes
- Review medical records and documentation for accuracy and completeness
- Ensure compliance with HIPAA regulations and maintain patient confidentiality
Skills:
- Supervising: Ability to oversee the work of other healthcare professionals and provide guidance as needed
- Physiology knowledge: Understanding of human body systems and how they function
- Home care: Experience in providing nursing care in a home setting
- HIPAA: Knowledge of privacy regulations related to patient health information
- Anatomy knowledge: Familiarity with the structure and function of the human body
- Documentation review: Ability to review medical records for accuracy and completeness
- Nurse management: Skills in managing nursing staff and coordinating patient care activities
- Acute care: Experience in providing nursing care to patients with acute medical conditions
- Medical records: Proficiency in maintaining accurate and up-to-date medical records
- Leadership: Ability to take charge in critical situations and make informed decisions
