Guardian Angels Health Center is looking for a Licensed clinical social worker (LCSW) or a Licensed professional counselor (LPC) to join our practice. We are a small pediatric and family medicine practice in Aurora, Colorado with a large need to integrate behavioral health into our clinic.

Responsibilities:

Work together with primary care and pediatric providers in an integrated care model

Have own schedule of patients that are in need of behavioral health services

Provide diagnostic assessments, develop interventions and treatment plans as well as follow up care for patients

Be competent in counseling, crisis assessments, determining the need for appropriate referrals

Be able to appropriately document patient visits with EMR system

Requirements:

Master's degree

Credentialed with Health First Colorado Medicaid (highly preferred)

Ability to provide behavioral health services in an integrated primary care model

Bilingual not required but highly preferred (English - Spanish)

32-40 hours a week, open to 8hr shifts 5 days a week, 8-10hr shifts 4 days a week - this is open to discussion

Job Types: Full-time, Part-time

Pay: $36.00 - $43.00 per hour

Expected hours: 32 – 40 per week
