General Duties and Essential Functions:

1. Client Care as a Therapist:
   • Provides an appropriate level of care as determined by client need and in compliance with generally accepted standards of care.
   • Evaluates, plans, supervises, and executes an appropriate therapy treatment plan for assigned patients per State, Federal, and RR guidelines.
   • Maintain a current knowledge base with regard to state certification regulations and licensing requirements that are Standards of Practice for the state, the Codes of Ethics for the specific discipline and assistant supervision guidelines.
   • Communicates with referring physician and other professional/non-professional personnel regarding patient’s total treatment program.
   • Participates at care conferences as needed and directed by the Rehab Director.
   • Provides instruction to family and caregivers for as indicated.
   • Willing to assist other rehabilitation departments as needed to ensure client needs are met for all

Agency sites.

• • Responsible for maintaining 70% productivity on a monthly basis.

1. Documentation and Billing
   • Ensures accurate and timely compliance with State, Federal, Facility and Agency guidelines for daily documentation.
   • Ensures billing entry is done accurately on a daily basis.

1. Responsibilities
   • Schedules treatment appropriately to ensure so needs are met in compliance with the Agency productivity requirement.
   • Participates in departmental meetings, projects and program review efforts.
   • Assist in reporting department and/or family concerns and offer suggestions to improve programming when appropriate.
   • Participate and monitor aseptic cleaning of equipment as needed to ensure infection control policies are observed.
   • Protect Personal Health Information (PHI) for all patients assigned to them and follow applicable HIPAA guidelines to ensure privacy at all times.
   • Participate in training of new department personnel as directed by the Rehab Director.
   • Supervises, assigns tasks to, and directs the conduct of assistants within the State licensure guidelines and Federal Medicare program requirements.
   • Responsible to abide by company policies and procedures.
   • Demonstrates a positive and professional attitude to all facility staff, RR staff and family members to reflect RR’s philosophy of operation.

1. Other Functions
   • Flexibility to work in other treatment areas may be expected based upon company needs.
   • Other duties as assigned by the Rehab Director or the Leadership Team.

Qualifications

Qualifications:

• Graduate of an accredited school with a minimum of a bachelor’s degree in Physical Therapy, Occupational Therapy, or a master’s degree in Speech-Language Pathology.
• Completion of a clinical affiliation(s) as required by educational institution.
• Certification or license as required by the State of Wisconsin or applicable state to practice as a Physical Therapist, Occupational Therapist or Speech-Language Pathologist.
• Willingness to serve as a supervisor for an occupational therapy assistant (OT only) or a physical therapist assistant (PT only).

Physical Characteristics:

• Have ability to lift at least 40 pounds using appropriate biomechanical techniques, walk, bend, stand, and reach. Get on and off the floor, pick up or guide children on/off various surfaces, support adult clients in homes, community, or clinic
• Have ability to speak, hear, and see (near and far) sufficiently enough to evaluate, diagnose, and monitor patient needs, maintain accurate records, recognize people, provide directions, drive to various sites, identify pieces of equipment, and communicate with patients and team members.
• Have fine motor skills to effectively and efficiently handle therapeutic equipment and document treatment provided on electronic devices.
• Have endurance to work eight-hour days with a half hour lunch break. Ability to participate in sustained activity for up to four hours at a time.
