• General responsibilities include the provision of nursing best practices to assist individuals being served in
• our outpatient medication clinic. Assists those we serve in improving skills to lead healthy, self-sufficient productive lives.
• · Collects medical information (current and history) about the people we serve through on-going assessment, observation and analysis and documents accordingly.
• · Administers prescribed medications and treatments in accordance with approved nursing guidelines.
• · Obtains vital signs to detect deviations from normal and documents the client’s current status.
• · May lead wellness groups, medication education groups, and/or other medical related education groups.
• · Provides medication education to clients / members and their families.
• · Assists clients and clinicians, as needed, with health care and services plan development.
• · Assists with coordinating care with external medical providers as needed, such as the FQHCs, Pharmacies, PCPs, etc.
• · Can demonstrate knowledge and practice of effective crisis intervention skills.
• · Can demonstrate knowledge of the role trauma plays in recovery and provides trauma informed, gender sensitive, age appropriate, culturally competent, and person-centered services.
• · Can dialogue with those we serve about their view of recovery and what that means to them and how to utilize those skills in their aftercare.
• · Can demonstrate a working knowledge and skills in the following, at a minimum: developmental life span; State laws for reporting abuse and neglect.
• · As PRN, must be willing to provide nursing services at any GC location including outpatient, CSU, and Care-A-Van.
