Title: Family Support Provider (FSP) Department: Family Wellness Ctr.

Job Summary

Provides care coordination, comprehensive transitional care, individual and family support, and referrals to community and social supportive services to members of our SOC Services Team. The services provided will include mediating between the families and the professional support providing agencies in their lives and coordinating between all program resources identified and provided. Assiststhose we serveto gain knowledge and skills to reduce problems and improve skills to lead healthy, self-sufficient productive lives.

Position Requirements

· Must demonstrate knowledge & applicability of federal, state and local laws, regulations, and standards as they apply to the services we perform. This includes HIPAA security and privacy laws.

· Must maintain all required certifications, licenses, and / or accreditations as needed to fulfill job requirements.

· Must be willing to accept evaluation of services performed and make adjustments accordingly and in a timely manner.

· Must complete all mandatory and required trainings, on-time, upon orientation and annually thereafter.

· Must be willing to provide in-service education / training for others, if asked.

· Must be willing to participate in the agency’s Performance Improvement / Quality Assurance program.

· Most be able to use our electronic medical record system. This work is repetitive, requires long periods of sitting and/or standing and typing and must be done daily.

· Must be able and eligible to drive an agency vehicle, possess a valid driver’s license, current vehicle insurance and pass a MVR.

· Must be able to pass an OSBI background check.

· Must be able to move up to 50 pounds.

· Must be willing to perform other duties as requested by your immediate supervisor or members of the agency’s Leadership Team.

· Ability to maintain regular, punctual attendance consistent with the ADA, FMLA and other federal, state and local standards

· Understands due to CMS mandate all GCBHS employees must be fully vaccinated against COVID19, unless an exemption for medical or religious exemption has been granted.

· Requires High School Diploma or equivalent.

· Must complete Wellness Coach and WRAP 101 Training by the ODMHSAS.

· Must complete certification status as a Family Support Provider by the ODMHSAS.

Essential Job Functions

· Provides care coordination, comprehensive transitional care, individual and family support, and referrals to community and social supportive services to members of our Specialty Services team.

· The services provided will include mediating between the families and the professional support providing agencies in their lives and coordinating between all program resources identified and provided.

· Assists those we serve to gain knowledge and skills to reduce problems and improve skills to lead healthy, self-sufficient productive lives.

· Understands and can demonstrate knowledge of the primary functions of the CCBHC program (comprehensive care management, care coordination, health promotion, comprehensive transitional care, individual and family support services, and referral to community and social support systems).

· Guides clients into the development of skills and strategies for dealing with their problems.

· Assists in the development and implementation of service plans and strengths assessments and collaborates with other staff and clients in goal setting and discharge planning.

· Can demonstrate knowledge in the role trauma plays in recovery and provides trauma informed, gender sensitive, age appropriate, culturally competent, and person-centered treatment.

· Can dialogue with the people we serve about their view of recovery and what that means to them and how to utilize those skills in their aftercare.

· Can effectively work with primary care providers for the integration of care.

· Can demonstrate knowledge and practice of effective crisis intervention skills.

· Can demonstrate a working knowledge and skills in the following, at a minimum: developmental life span; State laws for reporting abuse and neglect; safety planning; and service planning.

· Understands case management / care coordination and the referral, linkage, and advocacy process.

· Is committed to utilizing evidenced based practices and interventions.

· Participates in the Oklahoma Families First Program and Celebrating Families (Circle of Security) treatment models as indicated and/or needed.

· Must become familiar with resources across the state to assist with treatment and discharge.

· Conducts 15-minute checks / one-on-one checks / and / or line-of-sight checks and records.

· May assist in the preparation of meals and documents meal / snack / liquid consumption as requested by the nursing staff.

· Assists in re-directing / calming upset clients using agency approved techniques.

Supervisory Relationships

Works under the direct supervision of the Family Wellness Center Supervisor.

Job Type: Full-time

Pay: From $17.50 per hour

Expected hours: No more than 40 per week
