Job Description:
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team in providing exceptional personal care services. The ideal candidate will play a crucial role in ensuring the well-being and comfort of our residents/clients while upholding the highest standards of care.
Responsibilities:
- Deliver personalized care to residents based on individual needs and care plans
- Perform nursing assessments and assist in developing personalized care plans
- Administer medications and treatments as prescribed by healthcare professionals
- Monitor residents/clients' health status and promptly address any changes or concerns
- Collaborate with caregivers, families, and healthcare professionals to ensure comprehensive care
- Provide education and support to residents/clients and their families regarding health conditions and care plans
- Maintain accurate and updated documentation of care provided
Qualifications:
- Valid LPN license in Pennsylvania
- Experience in personal care, assisted living, or a similar setting preferred
- Strong knowledge of nursing principles and practices related to personal care
- Compassionate and empathetic approach towards residents/clients
- Excellent communication and interpersonal skills
- Ability to work effectively in a team environment
