Registered Nurse (RN) - Hospice Admissions. Day shift - flexible hours! Opening available in the Lakes Region.

The Granite VNA Hospice Admission Nurse (RN) is responsible for admitting eligible patients to Hospice Community Services at Granite VNA. Provides nursing assessment, initial plan of care and intervention(s) that optimize patient and family directed comfort and quality of life and ensures a smooth transition. This position may also provide follow-up visits to community patients when appropriate.

Obtains initial and ongoing physician orders according to agency policy for interventions and treatment.

Follows agency and program policies and procedures, including but not limited to:

• Admission Policy & Procedure
• Priority of visits
• Physician Certification/Recertification of Plan of Care
• Clinical Record Format & Documentation Policy
• Medical Policies & Nursing Standing Orders
• Home Infusion Therapy
• Procedure for Hospice Nurse in Evaluating Patient's Change in Status
• Declaring death
• Discharge Policy

This is a field position. Primary work location is in the community providing direct patient care not eligible for remote or work from home. Work assignments include holiday, weekend and/or on-call rotation.

* Eligibility - employed 20 hours or more a week

Requirements

RN Minimum Qualifications and Requirements

• Graduate of an accredited school of nursing, accompanied by current New Hampshire (RN) nursing license, or compact state license through the Nurse Licensure Compact (NLC).
• Three to four years of recent general medical/surgical or skilled/acute care nursing experience.
• Certification in Hospice and Palliative Care preferred
