Under general supervision, Registered Nurse, Home Care provides skilled nursing care to patient in the patients home and demonstrates competent knowledge and skills to assure care is appropriate to the adult and geriatric patients served. The Registered Nurse understands and employs professional nursing skills consistent with standards of Nurse Practice Act of PA and Grand View Home Care's policies and procedures.

Essential functions:

• The Registered Nurse, Home Care possesses and practices appropriate physical assessment skills, including assessment of the adolescent, adult and geriatric patient, family and home situation.
• Makes initial evaluation visit.
• Provides ongoing assessment of patients condition and nursing needs.
• Prioritizes patients problems or needs according to urgency to reflect the physical, psychological, emotional and spiritual aspects of care of the patient.
• Completes OASIS assessments per Medicare guidelines
• Fulfills on call responsibilities when assigned.
• Acts as a case manager to coordinate all home care services. Supervises Home Health Aides.
• Clarifies and implements physician orders and nursing care plan according to established standards.
• Performs medication reconciliation and instructs patient/family in medication use and precautions.
• Develops and revises an individualized plan of care and sets mutually acceptable goals with patient/patients family.
• Documents visits in Homecare Homebase.

Grand View Health's Home Care department serves Bucks and Montgomery communities. GVH Home Care strives for excellency by assisting patients with their health needs in the comfort of their home. The Home Care staff is expected to commute within a 15-mile radius of Grand View Hospital. Gas mileage will be reimbursed.

Education and Experience: Vocational diploma Nursing Diploma or Associates Degree minimum requirement1 year of nursing experience required
