Responsibilities:
- Provide personal care and assistance to individuals in their homes
- Assist with activities of daily living, including bathing, dressing, grooming, and toileting
- Monitor vital signs and report any changes or concerns to the Nurse Case Manager
- Assist with mobility and transfers, using proper lifting techniques and equipment
- Follow care plans and communicate effectively with the healthcare team
- Provides report and notes on a timely manner
Experience:
- Previous experience in a similar role, such as a nursing home, home health or hospice agency, is preferred
- Proficiency on using HospiceMD is a plus
- Knowledge of medical terminology and basic healthcare procedures is beneficial
Skills:
- Ability to lift and transfer clients safely and with proper body mechanics
- Proficiency on making notes on HospiceMD
- Strong communication skills to effectively interact with clients, families, and healthcare professionals
- Knowledge of infection control practices and ability to maintain a clean and safe environment
- Compassionate and patient demeanor to provide quality care to individuals in need
- Ability to follow instructions and work independently while adhering to established care plans
