Overview:
We are seeking a compassionate Care Coordinator to join our team. The ideal candidate will have experience in direct support, medication administration, memory care, dementia care, and senior care. If you are passionate about providing exceptional care to seniors and individuals in need, we would love to hear from you.

Responsibilities:
- Coordinate and oversee care plans for individuals in need of assistance
- Provide direct support and assistance with activities of daily living
- Administer medications following prescribed guidelines
- Assist with memory care and dementia-specific needs
- Ensure a safe and clean environment for all residents
- Collaborate with social workers and healthcare professionals to optimize care plans
- Lift and transfer residents as needed

Experience:
- Previous experience in caregiving or related fields
- Familiarity with senior care practices
- Ability to provide compassionate and attentive care
- Strong communication and interpersonal skills
- Knowledge of best practices in memory care and dementia support
- Ability to lift and assist residents with mobility
