Overview:
We are seeking a compassionate Care Coordinator to join our team. The ideal candidate will have experience in direct support, medication administration, memory care, dementia care, and senior care. Responsibilities:
- Coordinate and oversee care plans for individuals in need of assistance
- Provide direct support and assistance with activities of daily living
- Administer medications following prescribed guidelines
- Assist with memory care and dementia-specific needs
- Ensure a safe and clean environment for all residents
- Collaborate with social workers and healthcare professionals to optimize care plans
- Lift and transfer residents as needed

Experience:
- Previous experience in caregiving or related fields
- Familiarity with senior care practices
- Ability to provide compassionate and attentive care
- Strong communication and interpersonal skills
- Knowledge of best practices in memory care and dementia support
- Ability to lift and assist residents with mobility
