grace house is Hiring an Alternate Delegating Nurse (RN) Near Silver Spring, MD
Job Summary: The RN in this position would fill in for managing the clinical oversight at the Assisted Living Home when the regular Delegating Nurse/Case Manager is not available. This includes the delegating responsibilities over the CMTs and CNAs. They will work closely with the Delegating Nurse/Case Manager, the Director and Assistant Director to ensure the needs of the residents are met with quality and dignity. The Alternate DN would fill in on an as needed basis. We have small Homes but our residents have big hearts and are looking for that person who wants to be a part of their lives. Responsibilities:
Responsible for compliance with state, local and company policies, procedures and regulations
Function as the Delegating Nurse under Maryland State Regulations and be responsible for related tasks as required and indicated under the COMAR regulations
Coordinates and provides clinical oversight and care to the residents including medication management, pain management, wound care, nutrition, etc
Partner with the Assisted Living Director to complete all assessments and reviews as mandated by the MBON including: pre-admission, re-admission, significant change of condition assessments, 45 day reviews, and every 6 months state assessment reviews and yearly completion of new state assessment. Ensure that the resident's clinical assessment reflects the resident's preferences, goals and individualized needs. The DN will make recommendations to the team based on these reviews. Reports would need to be done on an as needed basis when the Alternate DN is filling in, to ensure that all reports are up to date
Ensure that the service plans are reviewed and revised at least every six months, but updated on an ongoing basis to reflect the needs of the individual resident
Coordinate and monitor outside support services such as Home Health, PT/OT and Hospice and ensure their treatment plans are being carried out and integrated into the resident service plan
Actively participate in meetings as designated and appropriate
Carry out in-service trainings for CMTs, CNAs and other staff if necessary in the absence of the regular DN/CM
Available to work day time hours during the week in order to be able to contact doctors and coordinate resident care with the assisted living administrative staff. May work other hours if needed and coordinated with the Director/Assistant Director
Willingness to be available 24/7 to support clinical care needs of the residents during those hours working as an Alternate DN
Qualifications:
Graduate of an accredited school of nursing with current licensure as an RN in the State of Maryland
BSN preferred
Successful completion of the MBON approved Delegating Nurse/Case Manager Course
Basic computer skills required: Microsoft Office, Internet and web applications
Experience with electronic medical records preferred
CPR/AED certified
A minimum of 5 years of experience as an RN required, with at least one year in assisted living
Previous experience as a Delegating Nurse in assisted living is preferred

Must be current on all vaccinations including COVID- 19 andreceive CDC recommended booster doses
