Job Description

Premier Home Health Care Services, Inc. is a leading established Home Health Care Agency. We currently have opportunities for HHAs within the New Brunswick, NJ area. You will be responsible for providing assistance in daily living activities, medical assistance, and other related services in a client's home.

• Works well with others in a team environment, as well as independently.
• Ability to assist clients with personal care activities and provide companionship tasks in accordance with the care plan.
• Comfortable working in an environment that can be humid and hot or dry, damp or dry, and has varying levels of lighting brightness.

Responsibilities:

• Monitor and maintain client's health condition by monitoring vital signs, taking measurements etc.
• Provide assistance in personal care services including bathing, dressing, grooming etc. as assigned, assist with mobility for light duty (light shower chair), light bed lift & mobility scooter as needed, feed and administer medications under the direction of the nurse/staff, assist with toileting as needed.
• Record all services provided per patient's care plan and on weekly/ monthly reports.
• Report changes in patient condition or behavior to the client or care team.
