Visiting Nurse Service of Ithaca and Tompkins County is Hiring a Home Health Aide Near New York, NY

Job Title: Home Health Aide

Position Description: Under the direction of the Director of Patient Services, and the field supervision of RNs and Therapists, the HHA performs personal care services for patients at their place of residence. HHA follows an established aide care plan to meet the medical needs of the patient. HHA must establish a good rapport and effective means of communication with patient(s) of varying abilities. HHA participates in a team approach to providing patient care and, as such, must establish good working relationships with co-workers.

Must be New York State certified. Visiting Nurse Service will pay for the required New York State certified home health aide training program that encompasses 65 hours of training and 3 hours of supervised clinical experience in patients' homes or equivalent training as described in the New York State Public Health Law.

Essential Duties and Responsibilities:

1. Bathes patients and provides personal hygiene care; assists patients with toileting.

2. Assists patients in performing therapeutic activities; ROM exercises prescribed by care plan.

3. Assists patient with administration of medication per care plan and NYS regulations.

4. Observes for changes in the patient's condition and reports these changes to the RN or Therapist.

5. Performs other appropriate HHA specialized care as instructed by the RN or Therapist on each individual patient (e.g. colostomy care, enema administration, heat/cold application, etc.).

6. Performs light housekeeping, makes bed, launders clothing and completes other tasks to provide a safe, clean environment in which the patient resides, within scheduled time frames.

7. Prepares meals for patients, unable to do so for themselves, and shops for groceries and other medically necessary items.

Other Responsibilities:

1. Uses telephone system to document the services provided and observations made; submits reports in a timely manner.

2. Must follow infection and exposure control protocols.

3. Meets with direct supervisor during scheduled times.

4. Attends required agency and departmental staff meetings.

5. Complies with agency policies and procedures.

6. Follows the Code of Conduct that must be reviewed and signed on an annual basis.

7. Undertakes special activities as assigned and other duties as appropriate. 8. Is proactive in maintaining a culture of safety in the work environment.

Physical Requirements:

1. Physical demands may include but are not limited to lifting, bending, standing and transferring patients. Must have the ability to lift and transfer patients using appropriate body mechanics and lifting equipment.

2. Mental demands may include but are not limited to emotions of grief upon learning of a decline in the medical condition or the passing of a patient.

The position requires a mixture of sitting (20%), standing (30%) and walking (50%). Individual must possess visual/auditory ability sufficient to comprehend written/verbal communication and the ability to clearly communicate to others. As well as:

  • Ability to perform tasks involving physical activity, which may include heavy lifting (the ability to transfer and position a patient that weighs 200lbs) and extensive bending and standing.
  • Ability to deal effectively with stress, both physically and mentally, while maintaining a professional attitude and reliably performing required duties.
  • Ability to work the hours necessary to accomplish job duties.

Qualifications:

1. Must possess excellent interpersonal skills.

2. High School Diploma or GED required.

3. Certification through a New York State certified home health aide training program that encompasses 65 hours of training and 3 hours of supervised clinical experience in patients' homes or equivalent training as described in the New York State Public Health Law. Must perform at a level of competence as defined by Federal and State Regulations.

4. Must pass a criminal background check, motor vehicle check and provide proof of a valid NYS Drivers License and current auto insurance certificate.

5. Must have reliable vehicle for transportation to and from work assignments.

6. Possess the ability to prioritize, organize tasks, and meet deadlines.

7. Must provide immunization record and physicians statement periodically.
