Being a UAS Nurse requires strong assessment and communication skills, as well as a deep understanding of various health conditions, disabilities, and community support services. We are looking for a dedicated, compassionate and person-centered nurse to help individuals thrive in their own homes and communities.
*UAS Nurse Responsibilities:*
* Perform UAS Reassessments in client's home or facility according to state guidelines.
* Document any concerns, conflicting information, other issues that surface during the evaluation process.
* Enter data into electronic UAS evaluation form.
* Assess eligibility in need of Long Term Care Services
* Ensuring compliance with regulatory requirements and healthcare industry standards
* Assisting in the coordination of interdisciplinary teams to ensure continuity and quality of patient care
* Providing education and guidance to patients and their families regarding healthcare resources and options available to them
* Reviewing patient medical records and assessing their clinical condition to determine the appropriate level of care
*UAS Nurse Qualifications:*
* UAS Certified (Uniform Assessment System for New York)
* Minimum 1 year preferred UAS / Home Care Experience
* RN License / New York State Nursing License
* Ready to work and dedicated

*Pay: $130 per reassessment visit (Fee for Service)*

Pay: $350.00 - $400.00 per day
