Job Description:

Anchor Home Health Care is seeking a dedicated and experienced UAS Assessment Registered Nurse to join our team. As a vital member of our organization, you will conduct comprehensive in-house assessments for our clients, ensuring they receive the highest quality care.

Responsibilities:

• Conduct 2-3 in-house assessments per day, 2-5 days a week for individuals interested in working with Arch Care.
• Perform any number of assessments for individuals interested in regular UAS assessments.
• Conduct in-house visits across various locations, including Bronx, Manhattan, Queens, Brooklyn, Staten Island, Putnam County, and Westchester.
• Utilize a Windows computer system for assessment documentation (MacBook not compatible).
• Communicate effectively with clients and multidisciplinary teams.
• Maintain accurate and detailed assessment records.

Qualifications:

• Hold an active NY RN license and possess an active NYS ID.
• Previous experience with UAS assessments is required.
• Proficiency in English is essential, and proficiency in Spanish, Russian, Mandarin, Cantonese, or Korean is advantageous.
• Flexibility to work varying schedules and locations.
• Strong interpersonal and communication skills.
• Ability to work independently and as part of a team.
