Job Description

Job Title: Service Coordinator

Division: Mission Services

Department: Intellectual Disabilities Services:Ho‘olana Program

Reports To: Program Coordinator / Director of IDS / VP of Mission Services

FLSA Status: Exempt

SUMMARY

Plans, coordinates, develops and directs curriculum, Individual Plans, supervised visits and activities for individual participants in Goodwill's H&CBS Ho‘olana Waiver Program services (ADH – Adult Day Health, CLS – Community Learning Services Group and Individual, PAB – Personal Assistance) by performing the following duties personally or through subordinates.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

Supports people with intellectual / developmental disability in a center and community-based settings. 

Develops, writes, and implements Individual Plans (IP) that include Support Plans, appropriate goals and objectives, task analyses, coaching strategies, specific methods, techniques, timelines and data sheets.

Maintains a case load of up to 35 participants.

Writes funder quarterly participant reports, completes monthly file audits and corrective action plans, and reviews data management system. 

Communicates and collaborates with Program Coordinator/Manager to ensure Individual Plans are current and person centered. 

Coordinates, schedules, and participates in participant planning meetings including Individual Plan (IP)/ Individual Service Plan (ISP) conferences, and special meetings.

Conducts monthly supervised visits to monitor waiver service delivery as outlined in ISP’s and IP’s.

Supervises direct support staff to ensure adherence to established standards of quality, participant satisfaction, and effectiveness of IP activities. 

Provides training, documentation, and feedback regarding individualized services in accordance with ISP’s, IP’s, and established policies and procedures.

Develops, organizes, and coordinates waiver service curriculum and activities in accordance with ISP’s and IP’s.

Promotes and ensures positive working relationships with participants, assigned employees, supervisors, other employees, members in the community, and participant support circle team members.

Communicates with parents, guardians, care providers, and support team members to ensure participant progress and quality of service.

Provides direct support to participants as needed when Support Specialists are unavailable for scheduled services.

Assists participants with personal care and hygiene as needed in accordance with individual needs.

Assists Program Coordinator/Director with service coordination, new admissions, suspensions, and terminations.

Manages participant budgets.

Displays and maintains a professional attitude at all times.

Maintains program, employee and participant confidentiality.

Communicates effectively and keeps Program Coordinator/Director abreast of team activities, actions, and concerns.

Complies with all safety standards, ensures the workplace is safe and clean, and utilizes protective clothing and/or equipment when appropriate. 

Complies with all company policies and procedures.

Works on special projects or other duties assigned by Program Coordinator/Director.

SUPERVISORY RESPONSBILITIES

Provides supervision, support, and training to Support Specialists. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, makes recommendations for hire, and training employees; educating staff on contract goals of the program; maintaining compliance with Goodwill policies and philosophy; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.

QUALIFICATIONS

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. EDUCATION and/or EXPERIENCE

Bachelor degree (B. A.) from four-year accredited College or university; and one to two years providing direct services to persons with developmental disabilities.

In lieu of a bachelor’s degree, Developmental Disabilities Division (DDD), will allow an exception for service supervisor qualifications when the following are met:

The Service Supervisor under an exception:

  • shall be on probation for the first six (6) months;
  • must be under the supervision/oversight of a qualified staff who will co-sign with the service supervisor while on probation; and
  • meets the following minimum requirements:
    • have a minimum of two (2) years experience working directly with individuals with intellectual and/or developmental disabilities;
    • has been employed by your agency for a minimum of six (6) months; and
    • possess the following: people skills; advocacy skills; ability to build relationships; and verbal and written communication skills.

LANGUAGE SKILLS

Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization.

MATHEMATICAL SKILLS

Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area, circumference, and volume. Ability to apply concepts of basic algebra and geometry.

REASONING ABILITY

Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

CERTIFICATION, LICENSES, AND REGISTRATIONS

Must have a valid driver's license and a traffic abstract that meet company guideline. Must have access to an insured vehicle with valid registration and safety check. Must be CPR/First Aid certified. Required to have TB clearance. Must have clearance of Criminal History Record and Adult/Child Protective Services Central Registry checks that meet Goodwill's standards.

While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand, walk, and sit. The employee is occasionally required to reach with hands and arms and stoop, kneel, crouch, or crawl. The employee must frequently lift and/or move up to 25 pounds and lift and/or move up to 50 pounds with assistance. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
