The function of this position is to provide on-site and off-site support with the daily operation and provision of services (including transportation) of the Job and Community connections program, which includes Individual Employment Support Services. Assist with the development and implementation of curriculum and activities consistent with best practices that include community engagement, vocational training, person centered planning, one on one coaching supports, and advocacy. Facilitate and participate in team meetings and activities that support individuals who receive vocational training and coaching supports. Assure that health, safety and welfare is protected for individuals who participate the vocational habilitation and coaching support program. Promote and enhance the involvement of guardians, family, community and other service providers. In addition, this position will support individuals to identify and connect to non-segregated/non-isolated environments and activities in the community based on their interests, abilities and conditions for participation. Assess and support the success and personal needs of individuals served by providing updates on progress and completing site visits to identify additional supports, needs, and training. Assist and support coaches and coordinators with identifying the personal, vocational, social and community interests, strengths and needs of persons served and, implement, monitor and evaluate program curriculum to meet these interests in conjunction with the Coordinator and Director.Assist and assess training needs to new and current coaches by providing on site supports as identified and as coaches are hired.Model behavior to improve independence and responsibility.Incorporate various teaching strategies utilizing available curriculum and encompassing all areas of life domains.Provide and explore vocational opportunities, including volunteer experiences in the community and with other community groups/members to enhance the success and experiences for the individual served.Assist with the management of the vehicles used by the program. Assures they are maintained in cooperation with fleet management arrangements, assures daily inspections, current licenses and tags are in place and that drivers are properly trained. Provide driver training, as authorized by GESMV, assist with scheduling and routing of vehicles on a daily basis as needed. Maintains own credentials to drive and drives, as needed to maintain program operation.Assist with daily operation of program and coaching as needed by managing a team of individuals with developmental disabilities in community settings.Establish and maintain on-going communication with parents, guardians, residential providers, county service coordinators and all other team members. Attend team meetings, when necessary, off site and outside of program day schedule.Make referrals, as assigned, to all professional consultants as deemed necessary by the participant’s planning team.Attends IP meetings regarding consumers overall plan and progress. Provides follow up, implementation and documentation to team as required.Provide additional assistance as needed with the staff during crisis situations and assures follow up and prevention plans after crisis situations with the additional director of the Coordinator or Director. Job Training/Coaching: Developing a systematic plan of on-the-job instruction and development by:• Supporting development of task analyses• Assisting individuals to perform activities that result in his or her social integration with other individuals and person employed at the worksite.• Training on application and resume development• Relationship building professionally and personallyParticipate in all staff meetings, regularly scheduled supervisory meetings and all mandatory in-services. Meet regularly with direct support staff to review procedures, scheduled activities, progress of participants and intervention and instructional strategies.Responsible for meeting state and federal requirements, preparing for audits and implementing new rules, as well as all program and agency policies and procedures.Assume other duties as directed by the Director of DD services and/or DD Supervisor IIComply with all Agency policies, procedures and safety standards throughout work areas and maintain same to meet CARF, HIPAA, Department of Labor and other regulatory standards. None Degree in Special Education or related Human Services field plus two years’ experience working with individuals with MR/DD required, prefer experience in teaching and or case management. At least 2 years demonstrated Supervisory experience required. Experience working with individuals with MR/DD required.
