Position includes weekend hours and at least one week day. Travel to provide care in patients private homes and residential facilities throughout Greater Boston and parts of Metrowest. The ideal candidate is well-organized and enthusiastic about working in a fast-paced environment. This position is eligible for a $5000 sign-on Bonus.
Job Description:
The Weekend Nurse is an integral part of a interdisciplinary hospice team responsible for caring for patients at the end of life. Makes home and hospital visits, provides information sessions, evaluates new patients, facilitates admissions to hospice services and coordinates care planning. Employs triage and assessment skills to titrate medications and manage symptoms appropriately and provides education to patients and families about terminal disease and end-of-life process. Communicates with a interdisciplinary hospice team about changes in patients needs, and outcomes of interventions.
