A Care Coordinator will coordinate the schedules and appointments between clients and caregivers. Care Coordinators will accommodate the needs of the client requesting schedules in order to distribute the number of hours allocated to determine services and find the time that works best for both the client and the matching caregiver. Care Coordinator will also manage shift adjustments and/or cancelations, finding the best alternatives for all involved.

• Answer phone calls, e-mails, respond to voicemails, greeting caregivers.
• Resolve scheduling conflicts as they occur.
• Attend to alert notifications regarding clients and caregivers shifts – alerts on call rotating system
• Attending to call outs
• Attending to all call centers, emails and voice messages in a timely manner
• Daily confirmations of shifts
• Adjustments to scheduling hours, not authorized by the client’s insurance agency.
• Document all communication details within the call centers or Supervisory Visits to keep a healthy communication flow within pertinent administrative staff.
• Shift coverage as needed

Skills and Qualifications:

• High school diploma or GED
• Excellent communication and organization skills
• Clerical or administrative experience a plus
• Knowledge of health care terminology helpful
• Basic computer program knowledge
• Bi-Lingual Preferred
• Work proficiently in a fast-paced environment

Pay: From $19.50 per hour
