Clinical & Support Options is Hiring a Care Coordinator, CP, Springfield Near Springfield, MA

CSO is looking for a Care Coordinator to join our Community Partner Program!

Be part of our team of caring and skilled professionals as a Care Coordinator in the Community Partner Program at Clinical & Support Options. We are a multidisciplinary team of Community Health Workers, Clinicians, and Nurses providing Care Coordination and support to adults with complex medical, behavioral health and substance use challenges. Through face-to-face outreach and telephonic care coordination, our team supports each person with their individual wellness goals in connection with their overall health. At CSO, we use a trauma informed and person- centered approach. Our Program provides supports across Franklin, No. Quabbin, Hampshire and Berkshire Counties. 

CSO offers Training in: Trauma informed care, Motivational Interviewing, Cultural Diversity, Equity and Inclusion, Mindfulness Skills and more. 

Responsibilities

  • Facilitate Care Plan Team meetings
  • Provide in person, telephonic and in-person outreach to members in their home and/or community.
  • Complete a Comprehensive Assessment and Care Plan for each Member. 
  • Reduce barriers to care through providing support to Member in accessing services. 
  • Provide wellness resources, and make referrals and linkages to community supports.
  • Maintain regular contact with Members. 
  • Assist members with system navigation.

Qualifications

Associate's or Bachelor's degree (Psychology, Sociology, Social Work, etc.) from an accredited college or university required (Bachelor's preferred.) Good listening skills, nonjudgmental ability to foster self-sufficiency and an ability to establish trusting relationships through compassion, empathy and insight. 1-2 years of experience is preferred. Capacity to be an effective member of a team. Demonstrated sensitivity to the needs and perspectives of people from diverse cultural or linguistic backgrounds. Computer proficiency required including ability to work with email, electronic health record and word processing software. MA driver's license and reliable transportation required.
