Job Overview:

We are seeking a dedicated Registered Nurse to join our healthcare team. The ideal candidate will have experience in case management, nursing home care, dementia care, internal medicine, infant care, primary care, discharge planning, and adherence to medical standards.

Responsibilities:

- Provide direct patient care in various healthcare settings
- Collaborate with healthcare professionals to develop and implement patient care plans
- Administer medications and treatments as prescribed by physicians
- Monitor and assess patient conditions and report changes to the medical team
- Educate patients and their families on health management and disease prevention
- Assist in discharge planning and coordination of post-hospital care

Requirements:

- Valid RN license in the state of practice
- Proven experience in case management, nursing home care, dementia care, internal medicine, infant care, primary care, and discharge planning
- Strong understanding of medical standards and procedures
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Compassionate and caring attitude towards patients
