A Day in the Life:

“I oversee the day-to-day operations of the Home Recovery Care team. I have an awesome team of registered nurses who monitor our Home Recovery Care patients. My team coordinates all necessary elements required for a patient to be cared for in their own home instead of a hospital. The goal is to deliver quality care and stellar outcomes while keeping the patient out of the hospital.”

Responsibilities:

As the Registered Nurse, you will be responsible for starting the patient’s care which includes performing a comprehensive nursing assessment, medication reconciliation, safety assessments, and implementing standardized protocols. The difference is that this care model is provided in the comfort of the patient’s home

• Perform patient assessment, provide care, educate patient on their condition, prescribed medications, nursing interventions, and care plan
• Participate in the coordination of services with other disciplines and community resources as needed
• Identify, act on changes in patient’s condition, evaluate and revise care plans/assessments based on changes in patients and/or their environments
• Collect specimens as required (including phlebotomy)
• Start and manage peripheral IVs, midlines, and PICCs
• Administer medications including IV medications as ordered
• Utilize and instruct patient/family on the use of the telehealth tablet used for biometric data capture, patient symptom reporting, and telehealth visits
• Participate in regular case conferences and in-service or special case orientations as needed and participate in on-call rotation

Schedules Currently Available:

• 12pm – 10pm EST – 1 weekend day required
• 7am – 5pm EST – 1 weekend day required
• 3pm – 11pm EST – no weekend shifts required

Compensation: $57-59/hour $3 shift differential after 7pm

Qualifications:

• Covid Vaccination REQUIRED*
• Current RN license
• 2 years of acute nursing experience (Emergency, Medical-Surgical, Intensive Care, etc.)

Job Types: Full-time, Permanent

Pay: $57.00 - $62.00 per hour
