Job Overview:
We are working with one of the top healthcare systems in the area. They are looking for nurses with hospice, acute care or home care experience. You will be seeing patients in their homes.

Duties:
- Conduct comprehensive assessments of patients' medical conditions and develop individualized care plans
- Coordinate and monitor patient care, ensuring timely and appropriate interventions
- Collaborate with physicians, specialists, and other healthcare professionals to develop and implement treatment plans
- Provide education to patients and their families regarding their medical conditions, treatment options, and self-care techniques

Skills:
- Valid Registered Nurse in the State of PA
- Minimum of 2 years of experience as a nurse in acute care, hospice or home health is preferred.
- Excellent communication skills, both written and verbal
- Must have proof of Covid Vaccine
- Strong organizational skills and attention to detail
- Proficiency in using electronic medical records systems

Schedule:

Monday-Friday: 8A-4:30P

Typically see 5-6 patients in their homes

No on call!!!
