The Georgia Department of Behavioral Healthand Developmental Disabilities (DBHDD) serves people of all ages who have anintellectual disability or defined closely related developmental disabilities(such as autism, cerebral palsy, and seizure disorder), and those withadditional dual diagnoses (ID and other psychiatric disorders).

Themain office for DBHDD is located in downtown Atlanta. DBHDD has regionaloffices throughout the state, including five regional hospitals

Job Description, Responsibilities,Standards and Qualifications

-Primary responsibility of a Levelof Care Nurse to complete Initial and Annual Level of Care Determinations forNOW and COMP waiver Participants.

-In addition, performs as a lead memberof the interdisciplinary intake and evaluation teams and provide nursingservices as assigned which includes reviewing and completing the level of careof individual support plans, writing assessments, updates, technicalassistance, and consultation.

-Assessments require conducting interviewswith individuals, family members, legal guardians, care givers, and primarycare physicians as needed.

-Administration of health risk assessment toolsand functional assessment tools are required for which training is provided.

-Responsibilities include making clinical recommendations and proposed actionplans to the individual support plan team that supports the personal goals ofthe individual and addresses their medical needs in the community setting.

Job Responsibilities &Performance Standards: Complete initial and annual Level of CareDeterminations for NOW and COMP waiver participants. 

Performs the following tasks on asneeded basis as assigned by Supervisor:

-Performs comprehensive assessmentsof individuals’ health care needs based on data collected through history,observation, or psychosocial examination, and analysis of diagnosticdata. 

-Performs complete physical and psychosocial/Nursing assessments onindividuals using all available sources of information, including individualand family medical records. Interviews’ individuals and/or families usingacceptable techniques that are appropriate for individual’s level offunctioning to obtain necessary information regarding medical, social and/oremotional background. 

-Performs appropriate routine tests and take vitalsigns (e.g. weight, height, blood pressure, urinalysis, hematocrit, pregnancytest, etc.).Thoroughly reviews individuals chart for background information andcorrectly identifies psychosocial stressors and level of functioning.

-Accurately documents assessment findings to produce a complete medical record(including psychosocial, medical, mental and/or social problems) on individualin a clear, complete and concise manner within established time frames. 

-Completes and files in individual’s chart a written Nursing assessment orupdates in accordance with established standards and within specified timeframes. 

-Monitors all aspect of client care, including diet and physicalactivity.

-Performs Initial and Participantdirected HRST assessments of individual’s health care needs based on datacollected through history, observation, basic physical and/or psychosocialexamination, and analysis of diagnostic data.

-As a part of required technicalassistance, observes directs individual care activities by observingmedications, observing and recording individual responses and medication sideeffect(s); make rounds and recommends interventions/treatments. 

-Consultsand coordinates with health care team members to assess, plan, implement andevaluate individuals care plan. 

-Provides customer service to variousindividuals that we serve, as well as family members, care givers, and supportpersons.

- Analyzes problems by evaluating available information and resources, developseffective, viable solutions to problems.

Performs various administrative,clerical and documentation activities to support Intake and Assessment Programactivities.

-Records individual care and other information on appropriate formsin a legible, clear and timely manner per established unit policy and standardof care. 

-Maintains and ensures confidentiality of individuals’ conditions andrecords, unit records, staff and other date on a regular basis. 

-Maintainaccurate and well-organized charts, files and individual records perestablished guidelines. 

-Prepares complete and accurate reports, meetingminutes, etc. per established procedure and/or formats and submits toappropriate personnel in a timely manner. 

-Participates in individualized andcomprehensive plans of care consistent with assessment, physicians’/clinicians’treatment plans, individuals’ level of need and functional ability asneeded.

-Provides health education instruction to consumers on varioushealth issues (e.g., treatments self-care, surgical procedures, etc.) 

- Provider in-service training for staff. 

-Assess individuals, health problems andneeds, develop and implement nursing care plans, and maintain medical records. 

-Advise clients on health maintenance and disease prevention of provide casemanagement.

-Provide clear and accurateinformation to consumers, families, providers regarding diagnosis, treatmentplan, discharge plan, and/or preventive care per individualized healthneeds.

-Respond appropriately to inquiries.

-Communicates with agencypersonnel, service providers, support coordinator and other interested partiesto coordinate effort, resolve operational problems and improve quality ofconsumer care. 

-Works cooperatively with agency personnel, service providers,support coordinators and other individuals involved in the care and serviceprovided to the consumer to maximize the appropriateness, benefit andeffectiveness of services.

- Accurately identifies measures evaluates and resolvedeficiencies found in the consumer care process in a timely manner. 

-Completeassessments as assigned. 

-Additional duties as assigned.

Bachelor's degree in nursing from an accredited college or university OR Associate's degree in nursing from an accredited college or university AND current Georgia license as a Registered Professional Nurse AND any related professional work experience.
