SOCIAL SERVICES COORDINATOR – Full-Time - Generations at Regency, Niles, IL
Generations at Regency in Niles, Illinois Seeks a Dynamic Social Services Coordinator. This is a Full-Time role with an 8-hour shift, and an Annual Salary of Between $55k and $60k, depending upon Experience.
Monday - Friday, 8:30am - 5:00 pm, with one Sunday rotation per month.

We provide a home-like setting plus concierge-level services and amenities for seniors in need of long-term care or short-term rehabilitation. Responsibilities of the Social Services Coordinator:
- Assist with planning, developing implementing our facility’s social service programs.
- Ensure that medical, emotional, and social needs of each resident is met/maintained
- Greet and orient new residents
- Provide information regarding community resources to residents' families
- Assist with discharge planning
- Attend regular care plan meetings with residents and their families
- Attend intervention, rehabilitation, marketing, performance improvement and Medicare meetings as appropriate

Requirements
of the Social Services Coordinator:
- Experience in the social services field
- Demonstrated knowledge of the social dynamic of the geriatric population
- Knowledge of government regulations as relevant to social services a plus
- Bachelor's Degree or Master’s Degree in Social Work (MSW) a plus, but not required
- A member of the National Association of Social Workers (NASW) and Academy of Certified Social Workers (ACSW) a plus
- Knowledge of community resources helpful
- Initiative to seek new methods and services for residents
- Strong relationship building skills
- High level of organizational professionalism
