Description

Pay Type - Salary (commensurate with years of experience at GHV)

ESSENTIAL RESPONSIBILITIES

1. Responsible for oversight of the implementation of individualized service plans for each resident. Responsible for ensuring the quality of services delivered. Lead monthly house meetings with residents to receive feedback on the quality of care received. Develop and implement program improvements to ensure continuous quality of care. Act as an essential member of the comprehensive Support Team planning process.

2. Responsible for management of medication administration. Ensure that all medication is properly administered and documented for each resident and that all scripts are up to date. Responsible for ensuring all staff are properly trained on agency policy and procedure for medication administration.

3. Responsible for ensuring the provisions of care are directly related to the individualized plan of nursing services for residents that require nursing care. Coordinate with the Manger of Health & Nursing Services to ensure continuity of care for all residents.

4. Uses independent judgment and discretion for hiring of program staff vacancies. Conduct interviews and notify Human Resources of chosen candidate. Responsible for training of new hires on all agency, department, and program policies and procedures.

5. Directs the work of program staff and delegates tasks as appropriate.
a. Responsible for the managements of the staffing pattern. Maintain the program schedule and ensure staff coverage per agency, program, and applicable regulatory guidelines. Responsible for coverage of shifts as needed.
b. Responsible for the training, development, and orientation of staff. Provide enhanced training to staff regarding specialized populations to ensure effective delivery of individual care.
c. Complete probationary and annual evaluations for all assigned staff. Notify Human Resources and Director of Residential Services of employee relations issues. Coach and counsel staff on performance issues and provide disciplinary notice as appropriate.
d. Lead monthly staff meetings to ensure continuity of care of residents. Receive feedback from staff and develop and implement program changes to ensure highest quality of care.
e. Responsible for ensuring all staff are up to date on all required trainings and certifications including Regulatory Training, CPR, and Medication Administration (AMAP) as needed. Maintain a list of staff trainings and schedule staff for renewals. Inform Director and Human Resources of staff that are not in training compliance.

6. Acts as an essential member of the intake and discharge process with the Movement & Benefits Coordinator. Conduct screenings of potential residents to evaluate needs and fit for the residential program and collaborate with the residential management team for individual selection into the program. Coordinate with potential residents and family members for trial visits and tours of the residence.

7. Responsible for oversight of all food and household supply orders to ensure all resident needs are met. Develop and implement controls to ensure quality and efficiency of food procurement, storage, preparation, and serving in accordance with resident individual needs.

8. Responsible for maintenance of assigned program budget and monthly billing to ensure quality of services delivered and program financial solvency. Review and submit monthly billing notes for reimbursement for delivered services. Ensure staff are properly trained on the writing and timely submission of billing notes for services given.

9. Responsible for all entitlement, financial, and program records.
a. Ensure residents' monthly benefit payments are received and tracked. Communicate with Finance as needed.
b. Monitor residents' finances to ensure accuracy. Implement controls to ensure protection and proper usage of finances.
c. Maintain records for all residents. Ensure all documentation is properly stored and destroyed in accordance with agency and regulatory compliance standards including HIPAA.
10. Responsible for ensuring that program rent is received from residents (or representative payees) each month in a timely manner.
a. Ensure that payment plans are put into place for any resident owing rent in arrears
b. Responsible for tracking monthly rent collections and following up accordingly

11. Responsible for ensuring the health, safety, and welfare of residents in accordance with agency and regulatory compliance requirements.
a. Ensure cleanliness of residence. Instruct staff on proper food and waste disposal and proper cleaning procedures.
b. Notify Facilities/Maintenance of needed maintenance and repairs of residence
c. Develop and implement controls to ensure resident safety in case of an emergency.
i. Ensure monthly fire drills are conducted and staff is properly trained on assisting residents in the case of an emergency evacuation.
ii. Ensure staff are properly trained and certified to provide CPR as needed
iii. Ensure food preparation is done in accordance with safety procedures and individual resident diet needs.
iv. Responsible for handling and reporting incidents. Train staff on policy and procedure on how to handle incidents (health, behavioral, etc) in the moment and during follow up.
v. Serve as an on-call emergency contact. Be available at all times for program staff to support in emergency situations.

12. Responsible for maintaining workplace compliance and regulation notices with the support of Human Resources.

13. Acts as a representative of Gateway in the community and liaisons with local, county, and state agencies to advocate for residents and the agency.

14. Function as a Qualified Mental Health Professional/Qualified Intellectual Disabilities Professional (QMHP/QIDP)

15. Provide direct care services to residents as needed.

Qualifications

REQUIRED QUALIFICATIONS

Experience:

  • 2 years' experience in Human Services field. 1 year of supervisory experience preferred.

Licenses / Certifications:

  • CPR/First Aid
  • Medication Administration (AMAP)

PREFERRED QUALIFICATIONS

Education:

  • Bachelor's Degree in human services related field preferred

BONA FIDE WORKING CONDITIONS

  • Must be able to sit or stand for up to 8 hours per day
  • Must be able to bend down or kneel occasionally (1-2 times per week)
  • Must be able to bend and use hands, wrists, and fingers with full range of motion
  • May occasionally be required to lift up to 30 pounds (1-2 times per month)
  • Must be able to perform CPR in emergency situation and for regular recertification

REGULATORY CLEARANCES REQUIRED:

  • Must have a valid NYS Driver's License and maintain a driving record which meets the insurable criteria of the Agency's automobile insurance carrier

  • Must have a favorable result from all state, federal, and agency mandated background checks
    • New York State Justice Center Fingerprinting Criminal Background Check
    • OWPDD MHL Abuse/Neglect History Check
    • Statewide Central Registry Check
