Description

SUMMARY: Responsible for the daily clinical and administrative management of the facility program and program activities. Develop and implement all program service components as defined by the Department of Social Services, Community Care Licensing Division, Title 22 regulations or specific County requirements or regulations. Provide and supervise the quality of mental health program services to residents with regard for their physical and mental well-being and needs. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential job functions. This is a full-time position with the option for medical, dental, and vision benefits, as well as 401K (matching) and PTO.

ESSENTIAL DUTIES AND RESPONSIBILITIES Must be able to perform each essential job function satisfactorily.

Direct Care and Case Management

  • In a culturally competent manner will provide services to all residents without regard to race, color, creed, national origin, marital status, sexual orientation, age, sex, religion, and handicap or payment status.
  • Perform direct service and case management for clients.

Program Development and Management

  • Develop and implement the mental health program and activities for residents.
  • Ensure each resident has an appropriate cost effective and efficient treatment plan, monitor their progress and provide outcome data.
  • Initiate, maintain and ensure effective interaction with other interfacing programs.
  • Review protocols, monitor program effectiveness, administer and evaluate resident, staff, family and other satisfaction surveys.
  • Initiate preventative measures and create intervention methods for clients or conditions that posing a threat of danger or crisis.
  • Inform individuals of their personal rights, responsibilities and Company’s grievance procedures. Ensure that services are compatible with residents’ freedom of choice.
  • Incorporate Title 22 requirements into program curriculum and facility management.
  • Other duties and responsibilities as assigned.

Facility Management and Maintenance

  • Ensure compliance of regulations, reporting procedures, and health and safety standards.
  • Assist in maintaining and auditing resident’s financial information and budgets.
  • Investigate thefts and losses at the facility and report findings to the Main Office.

Teamwork and Collaboration

  • Implement the treatment plan through coordination with physicians, coordinators, families and other providers. Provide information to clinical and other staff.
  • Provide information, assessment and enrollment in appropriate programs for persons needing or wanting additional health or mental health services.
  • Provide information on health and mental health care issues including prevention, early detection and remedial care to residents, families, other agencies and the community.
  • Provide provisions for the safety and guidance of residents with physical challenges.
  • Transport residents to appointments, outings etc.

Reporting and Documentation

  • Mandated to report any “suspected abuse of dependent adults and elders” directly to Law Enforcement, Adult Protective Services, Ombudsman, or Community Care Licensing.
  • Monitor residents’ progress and participation toward service plan goals, collect and report outcome data. Document coordination in progress notes and complete all documentation (E.g. annual assessments, service plans and progress notes) within required timelines.
  • Keep clear, accurate, and professional records in resident’s files, staff logs, and other required communications.
  • Meet individual billing expectations for Units of Service and work collaboratively to meet program requirements.
  • Report to the Supervisor on the operation of the program and provides necessary interpretations of recognized standards of care and supervision for the resident.
  • Submit timely and accurate program attendance, billing data, and other requested reports.
  • Comply with Internal Quality Control (IQC) standards.

Staff Supervision

  • Implement and monitor personnel policies and procedures. Document personnel incidents. Take a proactive approach to personnel issues.
  • Recruit, hire, orient and train program staff.
  • Supervise program staff; workloads, trainings, development plans, performance, evaluations, terminations, ensure clear lines of responsibility.
  • Accurately monitor and record staff’s timecards, payroll documentation, and training.
  • Provide supervision to interns.

Activities (As Designated)

  • Organize, conduct and evaluate planned activities
  • Give capable residents the opportunity to conduct or participate in the planning, preparation, clean-up and critique of the activities.
  • Encourage, direct and supervise the participation of volunteers in planned activities.
  • Post actives schedule readily accessible to clients, relatives, referral agencies. Maintain copies of these schedules for at least six months

Qualifications


QUALIFICATION REQUIREMENTS: Knowledge, skill, or ability required to perform the job.

Education and/or Experience

  • Bachelor’s degree in psychology, social work or related degree with of one year of program development and six months of staff supervision. OR
  • Graduate degree in related field and six months of staff supervision. OR
  • Three years in a community program providing direct services to persons with mental disabilities, six months supervising direct care staff.

Knowledge

  • Requirements for providing care and supervision of residents.
  • Knowledge and conformity to all applicable HIPAA, OSHA, state and local laws, rules and regulations including Department of Social Services, Community Care Licensing, Title 22.

Language Skills

  • Read, speak, write and understand English well enough to follow and share the regulations, requirements and communications for providing care to residents.
  • Able to effectively present information and respond to questions.

Mathematical Skills

Calculate figures and amounts such as discounts, proportions, and percentages associated with completing the requirements of the position.

Reasoning Ability

  • Ability to carry out written, oral, or diagramed instructions.
  • Competent in dealing with problems involving several variables in different situations.

Other Skills and Abilities

  • Timely complete tasks, meet deadlines, problem solve, and coordinate numerous activities. Adjust tasks in accordance with changing deadlines and priorities.
  • Maintain cooperative and professional working relationships.
  • Adequate computer skills to utilize e-mail, word processing and needed programs.

Additional Requirements

  • Must be at least 18 years of age.
  • If required to drive, must be 21 and accepted by the Company’s vehicle insurance.
  • Cleared by the Department of Justice and FBI prior to commencing work at the facility.
  • TB clearance within the last 6 months and yearly thereafter. Pass a health screening.
  • Maintain a valid First-Aid certification.
  • May be required to serve on company committees.
  • Complete all annual training requirements as mandated by Community Care Licensing Division, Title 22 and Front St. policies.
  • Attend mandated meetings and trainings; fulfill all coursework and training hours required
  • Complete Theft and Loss Prevention Program training within 90 days of hire.
  • Valid water safety certificate if applicable for facility.
  • Work in a safe and acceptable manner, following established safety procedures.

Physical Demands: Must be met to successfully perform the essential job functions.

  • Regularly required to talk, hear, and sit, stand for extended periods of time.
  • Frequently required to reach with hands, arms, stand, walk, stoop, and may occasionally be required to kneel, crouch, crawl or lift up to 50 pounds.
  • Be physically, mentally and occupationally capable of performing assigned tasks per Title 22 and other applicable regulations.

Work Environments: Employee may encounter while performing essential job functions.

  • Noise level is usually low to moderate.
  •  Possible contact with hazardous and caustic household cleaning chemicals.
  • The actions of clients may be unpredictable. Staff may be verbally assaulted and at times may be physically injured.
