COMPENSATION:

$67,000.00 per year to $171,600.00 per year payable biweekly commensurate on experience, credentials, and completion of assigned duties.

WORK SCHEDULE:

Monday Friday from 8:00 AM to 5:00 PM for full time. Three weekdays a week during normal business hours for part-time. This position may be worked in the office, remotely or in the community.

ANTICIPATED TRAVEL:

0% of the time. There is no additional compensation for travel.

SUMMARY OF POSITION RESPONSIBILITIES:

Based on the needs of clients, you will service 8 clients (4 for part time) per day for individual sessions and facilitate a group at the end of each work day. Time will be scheduled to document your service note after each session. Sessions are typically focused on mental health disorder needs for assigned clients Monday, Wednesday and Friday. On Tuesday and Thursdays, sessions are typically focused on substance use disorder treatment needs. Depending upon how local the provider is to the assigned caseload , the role can be totally in person, hybrid remote/in-person, or totally remote. The employee will be responsible for assessment of clients including the development, evaluation, and modification of the plan of care. The clinician interprets and performs complex patient care procedures specific to their area of expertise. Sets objectives and goals for treatment and develops treatment plans in conjunction with the medical director, implements and monitors treatment plans. Provides individual or group treatment, such as psychotherapy/counseling, supportive therapy or crisis intervention on a long term or short-term basis to various populations and age groups. Advise the program director on the development and implementation or rehabilitation services to assure that those services meet the needs of the individuals served by the Company. The employee shall provide medically necessary services to clients assigned to the employee by the Medical Director. The employee will work either from our main office via telehealth or in person. Employees are may also work in person with clients in our community-based offices local to where our persons served reside. Most persons served are chronically homeless and cope with co-occurring disorders. Employees are expected to understand evidenced based trauma informed therapeutic modalities. Model fidelity is also expected to be adhered to throughout treatment plan periods.

Responsibilities and Competencies

- Render individual, family, group counseling, assessments, evaluations, and treatment planning in frequencies as dictated by the client’s individual treatment plan. You may be required to be proactive in facilitating client conformance to treatment compliance. Failure to render services as indicated on the treatment schedule may result in disciplinary action. A service schedule template and scheduling assistant will be provided to assist employees with this responsibility.
- Schedule clients for service at least 7 days prior to appointment date and time. Issue appointment slips to clients. Mark appointment confirmation and outcomes in the calendar.
- Participate in the intake process as requested by the company.
- Initiate discharge process for client missed appointments. Rescind discharge initiation if clients conform within 1 day. Finalize discharge if client fails to conform within 1 day.
- Ensure all service notes conform to the checklist standard and are entered on the day services are rendered. Failure to document notes to standard may result in disciplinary action.
- Review and update individual treatment and rehabilitation plans (ITRP) during the first session of each month and as frequently as needed. Document the time spent with the client as an ITRP in ICANotes. Send the Revenue Cycle Management Department a helpdesk ticket to code each treatment planning session as a 90834 for 38 minutes spent with the client or a 90832 for between 16-37 minutes spent with a client.
- During the second session of each month, if the assigned client is enrolled in PRP, collaborate with client, and assign PRP worker. Document the session in ICANotes as a case management note. Code the session as a 90834 for 38 minutes spent with the client or a 90832 for between 16-37 minutes spent with a client.
- Provide clinical supervision to supervisees if credentialed and requested by the Company.
- Complete diagnostic evaluations on assigned clients every 6 months and code as 90791.
- Collaborate with the treatment team regularly as needed. Refer assigned clients to services the Company are unable to provide (such as methadone treatment, detox, inpatient) in collaboration with the treatment team.
- Request billing and authorization issue abatement through an urgent helpdesk ticket at the time an issue is realized.
- Have clients sign a log for all services received.
- Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client centered.
- Handle case assignments, draft service plans, review case progress and determine case closure.
- Help clients achieve wellness and autonomy.
- Facilitate multiple care aspects (case coordination, information sharing, etc)
- Help persons served make informed decisions by acting as their advocate regarding their clinical status, needs and entitlement options.
- Accompany persons served on interviews with agencies that provide entitlements as necessary.
- Develop effective working relations and cooperate with the treatment team throughout the entire therapeutic process.
- Take the extra mile and interact with persons served to keep track of their progress and to ensure satisfaction.
- Record cases information, complete accurately all necessary forms and produce statistical reports. Maintain the electronic file of all documents pertaining to entitlements coordination for each person served.
- Promote quality and cost-effective interventions and outcomes.
- Assess and address motivational and psychosocial issues.
- Adhere to professional standards as outlined by protocols, rules and regulations.
- Participate in weekly individual supervision / group supervision with a clinical supervisor.
- When scheduled for crisis duty, be available on-call to clients in the event of a mental health crisis; providing direct intervention over the phone, face to face, or via telemedicine where necessary. On the same day crisis service is rendered, document crisis intervention services rendered in the icanotes software and notify the supervisor.
- Assist supervisor with work related requests.
- Collaborate with and serve as program POC for external treatment providers and outside agencies as needed.
- Responsible for following regulations of COMAR, CARF, any other regulatory body, and company policies/ procedures related to service delivery and documentation and co-facilitating orientation of all new hires to these as well.
- Support the maintenance of a safe environment by participating in training and drills as requested.
- Maintain confidentiality of records relating to client’s treatment.
- Participate in external and internal audits/surveys (CARF/CSA/OHCQ) as directed by the supervisor.
- Participates in quality assurance and performance improvement plans by completing periodic audits or other activities to ensure regulatory compliance and/or improve service delivery.
- Ensure medical director or clinical supervisor review and co-sign documents as required.

Skills

- Proven working experience in counseling, including as a nurse, medical, mental health care manager or a related job

LEVEL OF EDUCATION/TRAINING/QUALIFICATIONS:

- Possess at least an associate degree in counseling, psychology, social work, or health related field (Interns and students pursuing such degrees may provide services under this scope through contract with the higher learning institution only)
- 1-year experience in case management, mental health counseling or substance use disorder related counseling

REQUIRED CERTIFICATION/LICENSE:

Must be licensed, certified or approved to render services Maryland in one of the following disciplines:
