The Care Management Assistant (CMA) provides support within primary care practices with guidance from the care management staff. Duties include: outreach to patients during transitions of care as well as outreach to patients to facilitate closing care gaps.
The Care Management Assistant will be knowledgeable on quality program requirements in regards to care opportunities. The CMA will also support the RN or Social Work Care Manager in patient centered care coordination. The CMA will assist patients enrolled in care management with accessing appropriate community resources in an effort to remove any barriers to care and to optimize self-management of chronic conditions as well as other duties to support the care management team.
An AA degree is preferred with 3-5 years of experience in healthcare setting preferred. Certified Medical Assistant or Certified Nursing Assistant certification required. Medical terminology, ability to work in a interdisciplinary team, care coordination and community resource navigation experience preferred.
