Mobile RN MDS Coordinator

We are seeking a highly skilled and motivated Mobile RN MDS Coordinator to join our dynamic healthcare team. Responsibilities:

1. MDS Assessment and Coordination:

• Conduct thorough MDS assessments for patients across multiple locations.
  Ensure accurate documentation of patient information and compliance with regulatory requirements.
  Collaborate with interdisciplinary teams to gather relevant information for MDS assessments.

2. Documentation and Reporting:

• Maintain detailed and up-to-date records of MDS assessments.
  Generate comprehensive reports on patient assessments and care plans.
  Communicate assessment findings to relevant healthcare professionals.

3. Quality Assurance:

• Monitor and evaluate the quality of MDS assessments to ensure compliance with industry standards.
  Participate in quality improvement initiatives to enhance overall patient care.

4. Education and Training:

• Provide education and training to healthcare staff on MDS assessment processes and requirements.
  Stay informed about updates and changes in MDS regulations and ensure staff compliance.

5. Mobile Coordination:

• Travel to different healthcare facilities to perform MDS assessments as needed.
  Maintain flexibility in schedule to accommodate varied locations and work environments.

Job Requirements:

• Valid Registered Nurse (RN) license.
• Proven experience in conducting MDS assessments in a healthcare setting.
• Strong understanding of MDS regulations and compliance standards.
• Excellent communication and interpersonal skills.
• Detail-oriented with a focus on accuracy in documentation.
• Ability to work independently and as part of a collaborative team.
• Flexibility to travel and work in different locations.

Preferred Qualifications:

• Certification in MDS coordination.
• Previous experience in a mobile or travel-based healthcare role.
• Familiarity with electronic health record (EHR) systems.
