JOB SUMMARY: Provides general social work services for FPHC, Inc. as part of a multidisciplinary medical team in a community health center setting. The Social Worker facilitates all aspects of the client’s access to needed services and resources countywide.
DUTIES AND RESPONSIBILITIES:
1. Provides Psychosocial assessment and risk identification to systematically determine the service needs of each client.
2. Develop individualized case plan to provide client centered care by identifying the action steps necessary to meet service needs.
3. Provides ongoing case management for assigned case load of patients served under the HIV program and Medicaid Maternity Waiver program. Follow-up via ongoing assessment, evaluated progress towards goals, amended care plan; as needed and by addressing issues of patient non-compliance.
4. Provides HIV pre-and post-test counseling.
5. Provide directly or through referral social case work, including individual/family crisis intervention and counseling, financial assistance including referrals to entitlement programs, medical services, prescription drugs, dental and vision services, information and contacts for transportation, shelter, legal, educational and vocational services.
6. Make and maintain excellent rapport with countywide social services agencies and work cooperatively with them for referral services HCH does not provide. Conduct follow-up on all referrals.
7. Document accurately and in a timely manner all client contact and services provided in the approved format.
9. Participates in FPHC, Inc Social Services Department activities of special interests in programs such as HIV, healthcare for the homeless, substance abuse, parenting programs, health education, EAP, etc .
10. Supply required data/reports to Director of Healthcare Quality or Program Director as required by program guidelines.
11. Provides supervision to clerical support personnel, special program volunteers and student interns. Prepares and conducts performance evaluation of subordinate staff and volunteers as designated.

QUALIFICATIONS:
EDUCATION AND RESPONSIBILITIES: Master of Social Work from an accredited school of Social Work with two or more years' work experience in a social services capacity. Possess licensed in the state of Alabama. Must have a valid Alabama Driver License.
KNOWLEDGE, SKILLS AND ABILITIES: Demonstrated knowledge of case management practices and skills, program development, referral sources, alcoholism and chemical dependency. Skills in establishing and maintaining effective working relationships with patients, medical staff, and the public are required. Skills are required in preparing and maintaining records, writing reports, and responding to correspondence. Must possess the ability to communicate clearly and maintain confidentiality. Ability to prioritize and organize case load.
CERTIFICATIONS, LICENSURE AND REGISTRATION: State of Alabama Social Worker License, valid driver’s license
