USA Health Systems is Hiring a Social Worker (PRN)- Care Management- University Hospital Near Mobile, AL

Responsibilities

Abides by and enforces all compliance requirements and policies and performs these responsibilities in an ethical manner consistent with the organization’s values; adheres to hospital policies including confidentiality; enhances professional growth and development through participation in education programs; requires minimal supervision to safely perform essential functions; utilizes proper body mechanics when: moving equipment/supplies that is necessary to perform essential functions; reviews initial patient screen that includes social, cultural, psychological, physical and financial needs per unit protocol; initiates consult within 24 hours of first working day in acute care; performs reassessment according to departmental policy; formulates appropriate and realistic discharge plans working with patient, family, other members of the health care team and available community based organizations; works closely with Care Coordinators and Utilization Review nurses as well as other health care team members to coordinate and implement the discharge plan; attends and participates in interdisciplinary patient care rounds or conferences; assists outpatients with crisis intervention and referral to community resources as needed; supports families and patients dealing with ethical issues, disability and terminal illness; ensures continuity of patient care by acting as a resource for patient information; provides after hours on-call services on a rotating basis; utilizes personal computer to access patient information and email system; communicates and uses appropriate customer relation skills with physicians, patients, families and healthcare team in person and via telephone; responds to overhead pages and digital pagers; communicates relevant aspects of the assessment and reassessment to patient, family & members of the health care team; maintains accurate and complete records by documenting: assessment, reassessment and plans in the patient’s medical record as dictated by hospital policy, maintains department copy file including correspondence with outside agencies as required; completes departmental activity report; accepts and completes all duties positively and without conflict; cooperates, helps others and improves the performance of the unit; completes all mandatory unit, educational and hospital requirements; utilizes cost effective practices in performing all aspects of the job; adheres to current Infection Control and Safety Standards; demonstrates the ability to provide competent patient care to the following patient populations: Neonate, Infant-Toddler/Preschool, School Age/Adolescent, Reproductive Age, Post/Peri Menopausal; participates on committees as assigned; participates in performance improvement activities as assigned; regular and prompt attendance; ability to work schedule as defined and additional hours as required; related duties as required.

Qualifications

Master’s degree in social work from an accredited institution as approved and accepted by the University of South Alabama; or Bachelors degree in social work from an accredited institution as approved and accepted by the University of South Alabama and two years social work experience; or Bachelor’s degree in a related field a from an accredited institution as approved and accepted by the University of South Alabama and three years social work experience. Licensure with the State of Alabama as a Social Worker.
