The Community Site Medical Case Manager plays a pivotal non-clinical role, serving as a compassionate advocate for patients living within Homeless Resource Centers in Salt Lake City. In this role, you will be working extensively off-site at the resource centers for the unhoused throughout the community, collaborating closely with the healthcare team to identify vulnerable patients, conduct thorough needs assessments, and build bridges to medical care for many individuals navigating the the realities of being unhoused. With a patient-centered approach, you will be creating and maintaining care plans tailored to individual needs, coordinating medical, behavioral health, and support services with both Fourth Street Clinic and community partners. Your dedication to facilitating continuity of care, accessibility to services, and addressing social determinants of health ensures that patients receive comprehensive support as they find their footing on their healthcare journey.

According to the U.S. Department of Health & Human Services, many people in the United States don't get needed health care services due to barriers such as a lack of transportation, affordable housing, educational background or discrimination. Fourth Street Clinic is here to provide services to vulnerable individuals and families, reducing barriers caused by social determinants of health. Fourth Street Clinic began in 1988 as a triage clinic with one part-time nurse, relying on hospital volunteers for patient treatment. Today, Fourth Street Clinic operates with a staff of nearly 100 individuals and an eight-figure annual budget. Last year, we provided comprehensive health care to 5,369 unsheltered individuals through more than 20,000 visits.
