The Case Manager plays a pivotal non-clinical role within the Most Vulnerable People (MVP) support team, serving as a compassionate advocate for patients in need. Working primarily at the MVP hotel in Sandy, they collaborate closely with the healthcare team to identify vulnerable patients and conduct thorough needs assessments. With a patient-centered approach, they create and maintain care plans tailored to individual needs, coordinating medical, behavioral health, and support services both within the clinic and with community partners. Their dedication to continuity of care, accessibility to services, and addressing social determinants of health ensures that patients receive comprehensive support as they navigate their healthcare journey.

This role is perfect for people who:

  • Possess strong interpersonal skills to conduct needs assessments for medically vulnerable patients.
  • Have experience planning and coordinating the delivery of medical, behavioral health, and support services.
  • Are detail-oriented and capable of maintaining timely, accurate case management data.
  • Have a deep understanding of community resources and can assist patients in accessing them.
  • Have additional experience or qualifications, such as a Utah State Certification for Case Management, or relevant bachelor's degree. (Preferred, not required.)
