Duties and Responsibilities :
- Increasing patient referral flow to our agency by building relationships with physicians, long term care, independent and assisted living facilities and other community resources.
- Identifying, evaluating, and prioritizing potential referral sources within the agency’s territory and surrounding area.
- Market to and build relationships with a variety of referral resources to offer services across multiple patient populations.
Qualifications :
- 2-3 years sales experience
- Knowledge of physician, hospital, skilled nursing facility, case management and discharge planning service needs
- Excellent organizational, interpersonal, presentation and communication skills
- Reliable transportation, travel required
- Bi-lingual preferred.
