We are looking for an excellent Social Services Director for our Skilled Nursing Facility. This is a great opportunity for a Licensed Practical Nurse (LPN) who is looking for a Monday-Friday position!

Job Responsibilities:

• Develop, implement, evaluate and direct the social service programs of the facility
• Participate in community planning related to the interests of the facility and the services and needs of the resident and family
• Maintain community relationships to help address resident concerns as appropriate
• Perform routine tasks with patient admissions, care plans and discharge planning
• Coordinate response to reports of missing, lost or stolen belongings.
• Assist in obtaining resources from community and social services agencies as well as health and welfare agencies to meet the needs of the resident.
• Develop and maintain a strong working relationship with other departments in the facility, and outside community agencies, so that social services can be provided to meet the needs of the residents.
• Document regarding resident social service status.

Qualifications:

• Licensed Practical Nurse (LPN) License
• Social Services experience preferred, but will train the right candidate!
