Experience/Education Requirements

  • Licensed Registered Nurse in the District of Columbia
  • Knowledgeable of nursing and medical practices and procedures.
  • Able to maintain a flexible work schedule in accord with the needs of the facility.
  • Has well developed organization and time management skills.
  • Experience with the MDS/RAI process and/or case management preferred

Job Type: Full-time

Salary: $90,000.00 - $110,000.00 per year

Purpose of Position

The primary purpose of this position is to manage the Resident Assessment Instrument (RAI) process of Forest Hills of DC. An additional and equally important purpose of this position is to assure the need of resident is clearly defined and outlined throughout the documentation in the MDS process.

I. DUTIES and RESPONSIBILITIES

Plans, implements, and evaluates designated MOS assessments in conjunction with the Care Plan Team ensuring quality of care areas are identified. Essential functions include but are not limited to:

  • Administrative
    • Ensure that the performance of all job duties are in compliance with the policies and procedures of the Methodist Home.
    • Serve on committees to review policies and make recommendations for revisions.
    • Participate in the development, maintenance, and implementation of facilities quality assurance program.
    • Participates in facility surveys made by authorized government agencies.
    • Performs administrative duties such as completing medical forms, reports, resident evaluations, studies, charting, etc., as necessary.
  • Minimum Data Set (MDS)
    • Opens a new MDS for each resident admitted to the healthcare center.
    • Gathers information from the direct care staff for the prescribed observation period.
    • Sends a schedule to the interdisciplinary team (IDT) listing the assessment reference dates (ARD) and the date for the care plan team and conferences.
    • Ensures that all information in the MDS is accurate for all disciplines as needed.
    • Tracks updates from the MDI system. Notifies the information systems liaison that an update has occurred and ensures that the systems at each station are functioning.
    • Enters discharge tracking information and re-entry tracking information.
    • Submits the completed MDS via the internet on a regular basis. Obtains transmission validation report.
    • Ensures that any significant change(s) are captured within the appropriate time frame.
    • Coordinates the completion of the RAP summaries for the IDT, writing them as necessary and appropriate.
    • Ensures that your knowledge concerning the Resident Assessment Instrument is current and that the facility is in compliance with Federal regulations.
    Manages the UR process, to include tracking of Med-A days, UR meetings with the IDT, and notifying families of the end of Medicare benefits.
  • Care Plans
    • Works with all disciplines to ensure that an interim care plan is in place within 24 hours of admission.
    • Conducts weekly care plan rounds.
    • Facilitates and prepares, following the care plan meeting, a resident-centered care plan for health care center residents following the IDT meeting. Makes changes to the care plan as necessary.
    • Lists the problem, measurable goal, and inter-disciplinary approaches for each resident. Evaluate the care plan at the specified time frame.
    • Captures special treatments on the care plan for each resident.
